A case of primary aldosteronism and concomitant pheochromocytoma occurring in the same adrenal gland is reported. The patient presented with abdominal pain, hypertension, and hypokalemia. Laboratory examination revealed hypokalemia, pronounced suppression of plasma renin activity, and elevation of plasma aldosterone level. The computed tomography scan of the abdomen showed a nodule on the right adrenal gland. Laparotomy revealed adrenal adenoma and pheochromocytoma in the same adrenal gland. Removal of the tumors was followed by resolution of the metabolic abnormalities and hypertension. The coexistence of two endocrine tumors in the same adrenal gland suggests a local interaction between adrenocortical and adrenomedullary areas. The possibility of a common etiology between two endocrine tumors is discussed.
