Hormone refractory prostate cancer remains a challenge, but it is not as resistant to treatment as previously believed. After failure of initial hormone therapy, various treatment options are available that may provide objective remission and palliate the symptoms of disease. Advances in molecular biology together with an increased understanding of the biology of the disease will direct future therapeutic strategies.
