The key to diagnosis is the ratio of lactate to pyruvate: if this remains normal, lactate elevation is of relatively small clinical importance; if lactate alone rises, the outlook is ominous. Occurring in the absence of any measurable fault in oxygen delivery or consumption, this type of acidosis may be less rare than is now supposed and may be incriminated in many unexplained sudden deaths outside the hospital.
