Two instances of virilization associated with pure granulosa tumors of the ovary are described. Both patients were 15 years old at the time of diagnosis. An unusual feature of both tumors was an extreme degree of cystic change, grossly suggesting a serous cystadenoma. This cystic variety of granulosa tumor previously has been reported only twice, and never in connection with virilization. An additional seven cystic granulosa tumors were found among the more than 150 gonadal stromal tumors with granulosa elements recorded in our files. One patient had hirsutism. All cystic granulosa tumors occurred in young patients, and all have proved clinically benign.
