If the finely tuned renal regulatory mechanisms for water balance go awry, the result may be either water deficit or surfeit. Clinical implications of the various states of water imbalance, from hypovolemic hypernatremia to hypervolemic hyponatremia, are related here to the disturbances of homeostasis they reflect, and the underlying Mechanisms are detailed to provide a theoretical base for rational management.
