The decision to perform valve replacement surgery is more difficult to make in cases of aortic regurgitation than in aortic stenosis. There is reluctance to perform surgery before symptoms develop but less chance of improvement when surgery is done later. The factors that must be evaluated and weighed in each patient with asymptomatic valve disease are described in detail.
