The increasing popularity of using breast milk for the sick infant has necessitated statement of a hospital feeding procedure. A major cause of concern when administering expressed breast milk is the potential of delivering pathogenic organisms to an already ill infant. Expressed breast milk, which is universally contaminated with maternal skin flora, has time-limited ability to suppress growth of these organisms. The bacteriostatic activity of breast milk is further compromised by storage procedures that are dictated by a regional approach to perinatal care that often separates infant from mother. This article presents rigid guidelines for providing bolus and continuous breast milk feeds, supported by rationale, that will minimize risk and maximize the beneficial aspects.
