SummaryA patient with acute lymphoblastic leukaemia developed bilateral pleural effusions without parenchymal lung lesions after administration of methotrexate. Repeated administration resulted in reappearance of the effusions. This syndrome may represent a less severe form of the known methotrexate associated pneumonitis. It may be due to a direct toxic effect of methotrexate or to drug hypersensitivity.
