AbstractAs increasing pressure forces hospital administrations into cost consciousness, emphasis is growing toward monitoring the outcomes expected of hospital services. Outcomes are monitored through documentation found primarily in patient records. Increasingly, chaplains are being asked to provide documentation of their activities, - an activity in which many chaplains have had little or no training. This article provides a foundation for, and methods that should be used by, chaplains who would document their work in patient records.
