Obstruction and reflux at the vesicoureteral junction constitute a very real danger to the kidney; much damage can occur silently over the long term in children with recurrent urinary tract infections, even if these are rendered asymptomatic by medical treatment. The indications for surgery and the necessary evaluative steps are discussed in the context of the case of a child undergoing ureteral reimplantation.
