Sports physicals probably do not differ appreciably from routine physicals in actual practice. The sports examination is a unique assessment requiring the careful attention of the health care provider to specific data that contribute to a more complete assessment of the child's readiness to participate in and compatability with a given sport. The tool designed herein is presented to assist each health care provider in making such assessments and to document pertinent findings.
