Recent advances in our understanding of the changes in blood as a result of storage, the role of leukocytes in the storage lesion, their clinical significance and minimization by leukocyte depletion are reviewed. The debate about a role for fresh whole blood is rekindled and the immunosuppressive effects of homologous transfusion are also reviewed.
