This essay discusses implant treatment options for the patient with edentulous maxillae. One option is the hybrid maxillary fixed implant-supported prosthesis. Evaluation of the smile line prior to treatment is advised to assess potential visibility of abutment cylinders. Coverage of the abutment cylinders with acrylic resin is possible. However, this significantly compromises oral hygiene access. A second option is the implant-retained overlay prosthesis which offers the advantages of hiding implant components and improved lip support, phonetics, and access for hygiene. A bar and clip can be used with two implant abutments to retain a maxillary denture with palatal coverage for partial support from the soft tissues. When four or more implants are placed, a bar and clip can be combined with posterior attachments. Although this allows for decreased palatal coverage, some tissue support may still be necessary. With five to six implants the prosthesis may be completely implant supported, but should not be cantilevered more than 10 to 15 mm. The third treatment option is the fixed partial denture design, using UCLA or Nobelpharma Estheticone abutments, which gives the appearance of natural teeth emerging through the mucosa. Buccolingual and mesiodistal dimensions are critical in implant placement for this prosthesis. A long-term acrylic resin provisional prosthesis is used to evaluate patient response before going to the final restoration. The patient must understand that an overdenture may be necessary.
