As various pathways have been followed in the search for the biochemical defect underlying the onset of failure, evidence has been obtained that changes in energy metabolism and in protein synthesis may contribute but do not play the primary causative role. A third, more promising, possibility is that the defect may be in excitation-contraction coupling, specifically in the role of calcium as a mediator of contraction.
