Persistent Extrauterine Pregnancy as a Result of Peritoneal Trophoblastic Implants After Radical and Conservative Surgery for Ectopic Pregnancy

BackgroundThe laparoscopic management of ectopic gestation is increasing in frequency, with an estimated 80% of these abnormal pregnancies amenable to this surgical approach. Persistent ectopic gestation occurs in 5–15% of conservatively managed extrauterine pregnancies and may be due to continued growth of trophoblasts at the tubal site or implantation in the peritoneum.CasesThe authors describe two cases of confirmed and presumed persistent extrauterine pregnancy due to peritoneal implantation of viable trophoblasts after conservative and radical surgery, respectively.ConclusionPeritoneal implantation may occur when viable trophoblasts are shed into the peritoneal cavity during retrieval of the tissue at surgery or sheds spontaneously after tubal abortion. The use of a disposable specimen pouch to retrieve the products of conception may reduce the risk of peritoneal implantation of trophoblast after laparoscopic surgery for ectopic pregnancy.
