Advances in transplant immunobiology are leading to new therapeutic modalities in clinical immunosuppression. Rational use of these new treatments will require an understanding of the emerging paradigms of alloimmune recognition and response. In this review we give an overview of the organization of the immune response to an allograft with a detailed focus on several topics that have seen significant advances over the past year.
