We report the recurrence of pneumothorax caused by ball-valving of an intrabronchial blood clot cast in a patient on mechanical ventilation. Removal of the blood clot cast by flexible fiberoptic bronchoscopy prevented further pneumothoraces. Bronchial obstruction should be considered in the mechanically ventilated patient with recurrent pneumothorax.
