Accurate and timely documentation of prehospital care is a challenge to many emergency medical services (EMS) systems. To provide meaningful system oversight and system improvement, certain data points need to be collected on all recipients of a 911 response. Collating these data points and interpreting the trends are important functions of the local EMS agency. The design of a computer-based data entry form for prehospital care is multifaceted and complex. Data points need to conform to local, state, and national requirements and provide some degree of freedom for free text entries. Data collection must be a coordinated effort between the hospital that provides the medical direction and the prehospital personnel who deliver the care. Implementing such a wide area network system that is on line, interactive, linked, and live has provided not only accurate and concise data but information that is immediately available to the caregivers and to the local EMS agency staff.
