SummaryPossibility of an overlooked ectopic pregnancy bas to be considered when instrumental uterine revision for early miscarriage does not bring out ovular debris. Of course it is most important to diagnose extra-uterine pregnancy as soon as possible in order to avoid later complications.The anesthesia gives opportunity for a vaginal examination but one has to bear in mind that a negative result of it does not exclude ectopic pregnancy. Abnormal bleedings after curettage, or spontaneous or provoked latero-uterine tenderness even in the absence of lateral tumefaction must keep the surgeon on the alarm.Probability of ectopic pregnancy is likely when histological examination of endometrial samples demonstrates clearcut decidual reaction and the absence of chorial villi. Absence of these histological findings does not exclude the diagnosis, especially if the examination takes place some time after the hemorrage. On the other hand it is well known that a decidua may devellop in the absence of pregnancy when there is hyperactivity of corpus luteum.In doubtful cases a positive Ascheim-Friedman reaction is satisfactory proof of ectopic pregnancy and justifies laparotomy. However A. F. reaction may exceptionally be slightly positive without pregnancy in rare cases of cystic corpus luteum.Uterine curettage must not be considered a routine technic for diagnosis of ectopic pregnancy. Risks for the patient do outweigh the obtained information.
