AbstractA series of 90 cases of proven myelomatosis has been reviewed to determine the incidence of sclerotic lesions in bone on the initial radiographs. Three such cases were found, and these are described. Case 1 showed multiple, well defined translucencies with sclerotic margins, in the pelvis, ribs, scapula and spine. Case 2 showed a dense area in a lumbar vertebral body which was probably not related to the myeloma and irregular sclerosis in a second lumbar vertebral body which probably was. Case 3 showed an area of sclerosis on initial radiographs of the cervical spine, and the same area four years later showed a destructive lesion.Reported cases of bone sclerosis in myeloma are reviewed and it is concluded that this is a definite, though uncommon, mode of presentation. It may not be peculiar to myeloma, but represent one way in which bone reacts to a slowly growing abnormal focus.Fourteen cases showed signs of neurological involvement, but the only case of peripheral neuropathy also showed bone sclerosis (Case 1). Previous reports of the association of these two lesions are quoted, but no explanation of its significance can be advanced.
