In brief: A 20-year-old wrestler sustained an injury to the right orbital area during practice. He did not lose consciousness, but he had headache on exertion and ecchymosis. Postconcussion headache was diagnosed one week after injury. Four weeks later he was admitted to the hospital with a severe, bitemporal, throbbing headache. A computed tomography scan showed a large left subdural hematoma. Surgery relieved his symptoms, and he was told not to participate in contact sports. This case demonstrates the need for careful and continued observation of all athletes who sustain concussions.
