Continuity of care is dependent on effective patterns of communication. Currently useful patterns of communications are nonexistent between the nursing staffs of tertiary medical centers and community hospitals. Nurses in community hospitals have difficulty obtaining discharge data regarding a child's admission to the tertiary center. In order to promote continuity of care between health agencies, Continued Help in Lending Direction (C.H.I.L.D.) has been proposed to assure a statewide telecommunication system between nurses at tertiary centers and community hospitals. Through the use of the hot-line, nurses in outlying hospitals will immediately be able to obtain information from the pediatric tertiary center if a child is later seen in the emergency room or admitted to the community hospital.This article proposes a communication network that would allow nurses working in hospitals throughout a state or region to have direct contact with a pediatric medical center through the simple process of dialing the telephone. The name of the proposed communication network is C.H.I.L.D. which stands for Continued Help in Lending Direction. The service would be available 24 hours a day. Information communicated through the system would allow nurses in community hospitals access to information which is needed to plan care for children with complex needs in the community. Information currently available on written referrals that have been sent to other health professionals wouldnolonger be lost or unavailable to nursing staff. The results of the initiation of this type of service include improved child care and a better informed nursing staff. While the continuity of care will be improved, it is hoped, the satisfaction of the family and the child with the health care delivery system will increase also. In addition, it should decrease unnecessary admissions and prevent complications related to planning which is based on inadequate data. Job satisfaction of nurses may increase as well.
