Characterized by heavy proteinuria and sometimes by progressive nephron loss, the nephrotic syndrome is now known to be not one disorder but several, some secondary, some idiopathic. Renal biopsy can identify the glomerular lesion and thus serve as a basis for selecting therapy and estimating prognosis. The four major lesions of the idiopathic syndrome are discussed in terms of clinical implications.
