Summary:Use of a hybrid bone graft consisting of bone marrow and blood mixed with a crushed cancellous allograft has been utilized in over 40 arthrodeses procedures in the foot and ankle over a l½-year period. The fusion rates appear to be equal to those of autograft, with less patient morbidity and the ability to obtain a much larger amount of graft material to solve difficult reconstructive situations. The technique is relatively easy. An incision roughly 1½″ long is made over the iliac crest. As much bone as possible is removed from a 1″ × 1-cm slot in the iliac crest, all of the bleeding bone marrow is saved, and 30-60 cc of a crushed cancellous allograft, depending on the amount of bone necessary, is fully saturated.
