Although angina is usually the key to diagnosis of significant coronary artery disease, many patients with myocardial ischemia do not experience chest pain. The prevalence, pathophysiology, and prognosis of clinically asymptomatic myocardial ischemia are reviewed, and diagnostic guidelines are discussed, as are management options, including medical prophylaxis and surgery.
