A new operative procedure for reducing the excessively high bridge of the nose, not by planing it down but by lowering it throughout its entire length, is described. Essential prerequisites are (1) Wegener’s basal osteotomy, which consists in removing the superior branch of the maxilla; (2) division of the nasofrontal suture; (3) longitudinal incision of the septum, parallel to the bridge of the nose; and (4) duplication of the septum. This procedure protects the cosmetically important bridge itself and avoids the drawbacks of Joseph’s kyphectomy. It is especially recommended when the whole bridge of the nose is too high.
