Epidemic dengue fever has been absent from the continental United States since 1945, but recent outbreaks in the Western Hemisphere have been too close for complacency. An understanding of the epidemiologic and pathologic dynamics that interact to escalate a self-limited disease to one of epidemic proportions as well as catastrophic clinical impact provides a basis for preventive strategy.
