AbstractParticipation in hepatitis B vaccination programs by dental health care workers (DHCWs) has been low. Following a vaccination program at the University of Michigan School of Dentistry, a survey was conducted to determine the relation of sociodemographic status, health beliefs, amount of patient contact, and perceived barriers to participation or nonparticipation in the program by 618 students, faculty, and staff. Stepwise logistic regression revealed that a respondent's age, perceptions about susceptibility to hepatitis B, amount of patient contact, and perceptions about the cost of the vaccine were significant determinants of participation. Findings from this study suggest that a highly structured program format is necessary to lower barriers and facilitate access to vaccination, while also cueing the DHCW to appropriate preventive behavior. Knowledge appears to be but one factor that may affect decisions regarding acceptance of hepatitis B vaccine; health beliefs, motivational factors, barriers, and program organization all have a significant impact on the DHCWs decision.

AbstractThe purpose of this investigation was to determine whether flossing, as an adjunct to toothbrushing, performed in a school‐based program can contribute significantly to a reduction in gingivitis. Four volunteer third grade classrooms (n=112) were randomly assigned to finger‐floss, looped‐floss, flossholder, and brushing‐only control groups. Measures taken at baseline and in four weeks included gingival (GI), plaque (PI), and flossing dexterity indices (FDI). Results using ANOVA showed no differences in PI among groups. However, both brushing‐only and finger‐floss groups showed GI scores significantly lower than the looped‐floss group. A repeated measures ANOVA revealed that the finger‐floss group improved gingivitis scores most over time, while the flossholder group improved scores the least. The ANCOVA results with FDI showed that at the final measurement, looped‐floss manual dexterity was rated significantly better than finger‐floss and that both groups were rated better than flossholder. AbstractThe prevalence of dental fluorosis in a nonfluoridated area was determined and related to the reported fluoride ingestion histories of the children examined. A convenience sample of 543 schoolchildren in rural areas of Michigan was examined for fluorosis using the Tooth Surface Index of Fluorosis. Questionnaires that asked about previous use of fluorides were sent to parents of all children examined. The response rate was 76 percent (412 usable questionnaires). A criterion for inclusion in the data analysis stipulated that only fluorosed surfaces that occurred bilaterally would be included. Fluorosis was found on 7 percent of all tooth surfaces and only in the mild form. Twenty‐two percent of the subjects were classified as having fluorosis. Dietary supplement was the only fluoride that was found to be significantly related to the occurrence of fluorosis. A greater proportion of the subjects with fluorosis fisted physicians, rather than dentists, as the source of fluoride prescriptions. The results demonstrate similarities to the fluorosis reported in other studies in non‐fluoridated areas, but also suggest the need to minimize the occurrence of fluorosis through proper assessment of a child's fluoride exposure and the judicious use of additional fluor

AbstractThis study investigated the prevalence of decayed, missing, and filled teeth (DMFT) in a state prison system, using the opening of a new institution as an opportunity to examine a cross‐section of the state male inmate population routinely. A representative sample of 178 male inmates was examined by a single dentist, and the results were linked to demographic information.This study found a mean DMFT of 10.5 for inmates aged 18‐29, 17.1 for inmates aged 30‐44, and 22.4 for inmates over age 44. In the 18–29 age group, white inmates had more filled teeth than blacks (P<.005) and more missing teeth (P = .06). Missing teeth increased by 0.54 teeth/year of age (P<.001) and DMFT increased by 0.66 teeth/year of age (P<.001). The number of decayed teeth was explained using negative reciprocal time incarcerated (−1/T), and declined by 1.30 teeth between the sixth and 12th month of incarceration (P<.01). This may have been due to a treatment effect, or by selective loss of the population with poorer teeth.
