AbstractThe slow adoption of sealants into dental practice is attributed partly to the fact that few prepayment plans pay for sealants. An important concern of insurers is how a sealants benefit will affect premiums.In this analysis, caries rates for various surfaces and teeth, failure rates for both sealants and amalgam restorations, fees and copayments, interest rates, reimbursement policies for replacement and repair, and the passage of time were studied to assess the cost to an insurer of amalgam restorations versus sealants. The results indicate:1. Sealants as a covered benefit for molars may not cause premiums to increase if fees and copayment levels are properly balanced. If caries in children continues to decline, however, higher premiums may be required if a sealant benefit is provided.2. Sealants as a covered benefit for bicuspids are likely to require higher premiums.3. The level of dental caries in the population should be monitored closely, because a further substantial decline in caries prevalence could diminish the economic argument for sealants.

AbstractThis paper describes the program structure and strategies being used by the Massachusetts Department of Public Health to promote the utilization of sealants. The program design includes four components: clinical demonstration, consumer education, professional education, and reimbursement. Eighteen Massachusetts neighborhood health centers and six local health departments are participating in the clinical demonstration component. Since March 1984, dental personnel from these sites have applied sealants to 4,398 schoolchildren. The promotional theme “Save Teeth: Seal Them” has been incorporated into brochures designed to increase knowledge and awareness of consumers. Curriculum materials have been developed to educate dentists and dental hygienists to apply sealants and understand the rationale and scientific basis for their use. Since January 1984, 18 sealant educational sessions have been conducted statewide for 630 dental providers. AbstractThis trend study reports the results of a survey designed to determine usage of dental sealants in Virginia. A total of 2,434 general dentists, dental hygienists, and pedodontists were sent questionnaires regarding usage, technics, products, educational experience, and public interest in dental sealants.Increases in the provision of sealant coverage and in public interest for this preventive measure were noted. Approximately three‐fourths of general dentists and dental hygienists working in a general or pedodontic practice are offering sealants to their patients. Pedodontists are the largest group of providers, with nearly 100 percent of pedodontists applying sealants. Recent graduates are applying sealants more often than earlier graduates. Preference for Delton™*clear color, light‐cured material was shown.In general, a lack of professional knowledge about sealants continues to exist. AbstractThe preventive outcomes of a comprehensive sealant program in a dental public health treatment center for low SES children are reported. The Linn County (Iowa) Dental Health Center implemented a comprehensive sealant philosophy in 1978. Since then, virtually all pits and fissures in primary and permanent teeth for all new and recall patients have been sealed. The comprehensive sealant philosophy was formulated following a previously reported Dental Health Center study documenting the negative outcomes of unmet treatment need as a result of the lack of continuity of care for this specific low SES target population.Sealants were evaluated for all recall patients seen over a two‐month period. The sealants had been placed six months to six years before the two‐month examination period. Ninety‐five percent of the sealants were intact for the recall patients. Only 3.3 percent of the sealants were in need of repair or reseating—a procedure easily incorporated into the preventive recall appointment. Only 1.5 percent of the teeth were carious, and to a minor extent. To maintain the total sealant treatment philosophy on repaired and newly erupted teeth for recall patients in the dental public health setting requires on the average approximately five additional minutes per recall appointment.Low SES children are at highest risk to dental caries, but are still not guaranteed access to ongoing preventive services. Sealants are of much greater importance than the usual prophylaxis‐fluoride regimen for low SES children displaying more episodic than preventive dental behavior. AbstractIn 1983, the New Mexico Board of Dentistry ruled that the New Mexico Health and Environment Department's sealant program violated its rules and regulations concerning the supervision of dental hygienists. The state was forced to reduce the number of children served by the program to comply with the decision. Attempts to secure a revision of the rules and regulations to accommodate the needs of public programs were unsuccessful. Relief eventually came through the revision of the Dental Practice Act itself to permit general supervision of all dental hygienists. The supervision controversy was an explosive political issue between dentists and dental hygienists that had negative repercussions for the state dental public health preventive program.

AbstractReferral directories of dentists willing to treat disabled individuals have been used as an expeditious means of improving access to care for this group. The purpose of this survey was to assess the usefulness of one such statewide referral directory. Agencies, individual field workers, and organizations to which the directory had been sent 18 months earlier were surveyed to determine the usefulness of the directory. Of the 77 groups and individuals surveyed, 46 (59.7 percent) reported that they had received the directory. The directory was considered “useful” or “very useful” by 58.7 percent of the acknowledged recipients. An additional 19.6 percent considered it to be “marginally useful.” The directory was estimated to be used at least once per month by 59.1 percent of the recipients.
