ABSTRACTSThe 24 editions of the professional journal ‘Speech’ published between 1935–1945 were examined by use of content analysis. The use of this approach enabled an in‐depth survey of the professional, clinical and political issues of the period which saw the beginning of speech and language therapy as a profession. Two main areas were focused upon: the development of the professional body and the authored papers which appeared in the journal. ‘Speech’ at that time can be viewed as a blend of today's ‘European Journal of Disorders of Communication’ with today's ‘Bulletin’.Les 24 numéros de la revue professionnelle ‘Speech’ parus entre 1935 et 1945 ont été examinés au moyen d'une analyse de contenus. Cette méthode a fourni un relevé en profondeur des problèmes professionnels, cliniques et politiques de cette période témoin des débuts de l'orthophonie en tant que profession. Deux domaines ont reçu tout particulièrement notre attention: le développement de l'effectif de la profession, et les articles signés qui ont paru dans la revue. L'on peut dire qu'à cette époque ‘Speech’ correspondait à la fois à ce que sont aujourd'hui le ‘European Journal of Disorders of Communication’ et le ‘Bulletin’.Die 24 Ausgaben der Fachzeitschrift ‘Speech’, die zwischen 1935–1945 erschienen sind, wurden mit Hilfe der Inhaltsanalyse untersucht. Dieser methodische Ansatz ermöglichte einen gründlichen Überblick über die beruflichen, klinischen und politischen Fragen jener Zeit, in der die Sprachtherapie als Berufsstand ihren Anfang hatte. Zwei Hauptbereiche wurden näher beleuchtet: Die Entwicklung der Berufsgruppe und die in der Zeitschrift erschienenen Aufsätze. ABSTRACTSStructured ‘communicative temptation’ procedures administered by a clinician were compared to unstructured parent‐child interactions in sampling intentional communicative acts in 11 subjects with intellectual disability. The subjects were assessed twice over a 5–7‐month period. The results indicated that the structured condition was more effective in sampling Requests and Comments, with more Requests than Comments produced. The unstructured condition was more effective in sampling responses, but only during the second assessment. Although there was no difference in the total number of intentional communicative acts produced across assessment times, there was an increase in the use of linguistic forms during the second assessment: that is, the subjects used more speech and signs and fewer gestures and general vocalisations during the second assessment than the first. The findings suggest the usefulness of combining structured and unstructured conditions in providing information on the variety of children's communicative acts and the linguistic level at which these are expressed.L'on a comparé des procédés structurés ‘d'incitation à la communication’ administrés par un clinicien à des interactions non structurées entre parents et enfants lors d'une collecte d'actes communicatifs intentionnels auprès de 11 subjets souffrant de déficits intellectuels. Les sujets ont reçu deux évaluations successives au cours d'une période de 5 à 7 mois. Les résultats indiquent que l'état structuré était plus efficace pour récolter des demandes et des commentaires, et que davantage de demandes avaient été produites que de commentaires. L'état non structuré n'était plus efficace pour la récolte de réponses que lors de la deuxième évaluation, Il n'y a pas eu de différence dans le nombre total d'actes communicatifs intentionnels d'une évaluation à l'autre; cependant on a assisté à un acroìssement de l'usage de formes linguistiques lors de la seconde, en ce que les sujets ont utilisé à ce moment‐là davantage de langage et de signes, et moins de gestes et de vocalisations générates. Ces résultats donnent une indication de l'utilité qu'il y a à combiner la structure et l'absence de structure pour obtenir des renseignements sur la variété des actes communicatifs des enfants et sur le niveau Unguistique de leur expression.In einer Stichprobe intendierter Kommunikationshandlungen von 11 geistig behinderten Kindern wurden strukturierte Verfahren zum ‘kommunikativen Entlocken’ von klinishem Personal durchgeführt und mit den unstrukturierten Interacktionen zwischen Eltern und Kindern verglichen. Die Kinder wurden zweimal während einer 5–7‐monatigen Periode bewertet. Die Ergebnisse zeigten, daß die strukturierte Bedingung für die Elizitation von Bitten und Kommentaren effektiver war; es wurden mehr Bitten als Kommentare ausgesprochen. Die unstrukturierte Bedingung rief insgesamt mehr Erwiderungen hevor, jedoch nur während der zweiten Bewertungsphase. Obwohl insgesamt die Anzahl beabsichtigter Kommunikationshandlungen während jeder der beiden Bewertungen gleich blieb, nahm der Gebrauch linguistischer Formen in der zweiten Phase zu; d.h., es wurden in der zweiten Phase mehr gesprochen und weniger Gesten oder unbestimmbare Vokalisierungen als in der ersten Phase verwendet. ABSTRACTSThis paper details the process of speech acquisition by the first British, congenitally deaf infant (without additional handicaps) to be fitted with a multi‐channel cochlear implant. The infant's phonologic and phonetic level development using, firstly, acoustic hearing aids and, secondly, a cochlear implant, is thoroughly detailed by use of video‐ and audio‐recorded data, taken at weekly intervals and across a variety of contexts. The paper examines the benefit of early implantation for spoken language development and notes the utility of Ling's model of speech acquisition for the habilitation of young, congenitally deaf implant recipients.L’ article décrit en détail le processus d'acquisition du langage par le premier enfant britannique sourd congénital qui a reçu un implant cochléaire à canaux multiples. L'enfant ne souffrait d'aucun handicap supplémentaire. Des enregistrements sonores et sur vidéos faits chaque semaine et recouvrant tout un ensemble de contextes ont permis de décrire point par point son développement phonétique et phonologique, au début grâce à des appareils acoustiques et ensuite grâce à l'implant cochléaire. L'article analyse l'avantage qu'il y a à procéder très tòt à des implants pour assurer le développement de la parole, et il examine l'utilité du modele de Ling de l'acquisition du langage pour l'habilitation de jeunes sourds congénitaux qui reçoivent des implants.Dieser Aufsatz beschreibt den Spracherwerbsprozeß des ersten britischen, gehörlos geborenen Kindes (ohne weitere Behinderungen), das ein mehrkanaliges Cochlearimplantat belommen hat. ABSTRACTSThere is little empirical research on speech and language therapists' use of counselling skills or their understanding of what counselling means. We report a survey examining both these issues and their relationship to work experience. Four groups (first and fourth year students, new and experienced therapists, total 151) completed a questionnaire measuring counselling and teaching work behaviours used by therapists, or anticipated by students. Significant differences were found. Scheffe's test showed: new therapists indicated fewer counselling behaviours than fourth year students and experienced therapists; fourth year students anticipated using less teaching behaviour than was reported by new and experienced therapists. This pattern may be partly due to new therapists adopting a defensive teaching role. No significant differences were found between the groups' understanding of counselling. Content analysis showed that whereas the therapist skills needed and the exploratory nature of counselling were identified by most participants, few mentioned enabling the client to take responsibility for change. This may relate to the perceived locus of control in the therapeutic situation. We discuss training implications and the role of personal therapy in developing a healthy therapeutic relationship.Il y a eu jusqu'ici très peu de recherches sur l'utilisation, par les orthophonistes, de techniques d'aide psychologique ou sur leur compréhension de ce que veut dire l'aide psychologique. Nous rendons compte d'une enquète qui examine ces deux aspects de la question et leur rapport avec l'expérience professionnelle. Quatre groupes (des étudiants en première et quatrième années, des orthophonistes nouvellement diplômés, et d'autres déjà expérimentés — au total 151 individus) ont rempli un questionnaire qui mesurait les comportements professionnels, en matière d'aide psychologique et d'enseignement, utilisés par les orthophonistes, ou encore prévus par les étudiants. L'on a trouvé des différences signifìcatives. Le test de Scheffe a révélé des faits suivants: les nouveaux orthophonists on fait état de moins de comportements d'aide psychologique que les étudiants en quatrième année et les orthophonistes expérientées. Les étudiants en quatrième année ont prévu d'utiliser moins de comportements d'enseignement que ce que les orthophonistes nouveaux et expérimentés ont dit qu'ils faisaient. Cette tendance est peut‐être due en partie au fait que les nouveaux orthophonistes adoptent un rôte défensif en matière d'enseignement. Du point de vue de la compréhension de Taide psychologue, aucune différence significative n'a été trouvée entre les groupes. Une analyse de contenu a montré que si la plupart des participants ont su identifier les techniques thérapeutiques nécessaires et la nature exploratoire de l'aide psychologique, rares sont ceux qui on mentionné le fait de rendre le client capable d'assumer lui même son propre changement. Ceci est peut‐être à mettre en rapport avec l'idée que l'on se fait communément du lieu d'où émanent les contrôles dans la situation thérapeutique. Nous discutons des conséquences du point de vue de l'entrainement, et du rôle de la thérapie personnelle dans le développement d'une relation thérapeutique suine.Der Gebrauch von Beratungsexpertise durch Sprachtherapeuten bzw deren Verständnis vom Begriff Beratung ist wenig erforscht. Wir berichten über eine Umfrage, die beide Fragen sowie ihre Beziehung zur Berufserfahrung zum Thema hat. Vier Gruppen (Studentinnen im ersten und im vierten Studienjahr, neue und erfahrene Therapeutinnen; insgesamt 151 Personen) beantworteten einen Fragebogen, der Einstufungswerte für das Beratungs‐ und Lehrverhaltensweisen ergab, die von Therapeutinnen angegeben bzw. von Studentinnen im voraus angenommen wurden. Es wurden signifikante Unterschiede gefunden. Nach Scheffé wurden von neuen Therapeutinnen weniger Beratung als von Studentinnen im vierten Studienjahr erfahrenen Therapeut Innen angezeit. Studenten im vierten Studienjahr sagten weniger Lehrverhalten voraus, als von neuen und erfahrenen Therapeutinnen berichtet wurde. Diese Verhallensmuster sind eventuell zum Teill Folge einer defensiven Lehrerrolle seitens neuer Therapeutinnen. Es wurden keine signifikanten Unterschiede zwischen den Gruppen im Verständnis des Begriffs Beratung gefunden. Eine Inhaltsanalyse zeigte, daß trotz Festellung der Fähigkeiten von Therapeuten bei der Beratung und der explorativen Funktion von Beratung durch die meisten Teilnehmer wenige von ihnen die Befähigung des Patienten erwähnten, die Verantwortung für eine Veränderung zu übernehmen. Dies hängt eventuell mit dem Verständnis der Kontrollinstanz in der Therapiesituation zusammen. Wir erörtern die Implikationen für Ausbildung und die Funktion

ABSTRACTSTwenty‐four medicated schizophrenic subjects participated in a study aimed at assessing language dysfunction in schizophrenia. Two groups of subjects participated: schizophrenic responders and non‐responders to treatment with antipsychotic medication. All subjects were tested on The Western Aphasia Battery test (WAB), the Cookie Theft picture descriptions task, and the Wechsler Adult Intelligence Scales — Revised (WAIS‐R) block design and picture arrangement subtests. All verbal and non‐verbal assessments were compared between groups. The three main findings of this study were:1. Severe language dysfunction among schizophrenic patients who do not respond to medication treatment. That is, therapeutic response to medication was the major predictor of the severity of language dysfunction in schizophrenia.2. A group profile of language dysfunction differed in severity but not in shape between responders and non‐responders to treatment with antipsychotic medication.3. Schizophrenic responders and non‐responders to medication treatment did not differ in their performance on a standardised picture description task and failed to reach low‐moderate aphasia level.Secondary findings suggest that non‐verbal aspects, such as attention and logical sequencing, may be influenced by treatment. This study represents an assessment of schizophrenic language function in relation to known language deficits of neurological patients, lending further support for the role of central nervous system (CNS) dysfunction in schizophrenia, and the importance of assessing language dysfunction as a sensitive gauge to treatment response.Vingt‐quatre schizophrènes sous médication ont participé à une étude destinée à measurer les dysfonctions du langage dans la schizophrénic. L'on apris deux groupes: des schizophrènes qui réagissaient favorablement aux médicaments, et d'autres sur qui les médicaments n'avaient aucun effet. Tous ont été testés grâce à la Western Aphasia Battery, aux épreuves de descriptions d'images du ‘Cookie Theft’, et aux sous‐tests du WAIS‐R Block Design and Picture Arrangement. Les groupes ont été comparés pour les notes obtenues en ce qui concemait d'une part le langage, et d'autre part les éléments extra‐linguistiques. Les trois principales conclusions de cette étude sont que:1. Des désordres sévères du langage apparaissent chez les schizophrenes réfractaires à la médication. C'est à dire que la réaction thérapeutique aux médicaments est l'indice prédéterminant principal de la sévérité des troubles du langage dans la Schizophrénie.2. Chez tel ou tel type collectif de trouble du langage, la différence en sévérité (le type restant la même) correspond au fait que les patients réagissent on non aux traitements antipsychotiques.3. La réaction ou l'absence de réaction aux médicaments ne crée aucune différence en cen qui concerne la performance lors d'une épreuve standardisée de description d'images, et dans ce cas tous les patients sont restées en‐dessous du niveau atteint par des aphasiques de niveau inférieur‐modéré.Les résultants d'ordre secondaire laissent à penser que des aspects extra‐linguistiques comme l'attention et le déroulement logique des idées peuvent être influencés par le traitement. Es nahmen zwei Gruppen teil: Schizophrene Reaktande und Nicht‐Reaktande bei antipsychotischer medikamentöser Behandlung. Alle Personen wurden mit der ‘Western Aphasia Battery’, der ‘Cookie Theft’ (Keksdiebstahl) Bildbeschreibungsaufgabe und den WAIS‐R Teilaufgaben ‘Block Design’ (Formentwurf) und ‘Picture Arrangement’ (Bildzusammenstellung) getestet. Beide Gruppen wurden in allen sprachlichen und nicht‐sprachlichen Bewertungen miteinander verglichen. Die drei Hauptergebnisse der Studie waren:1. Starke sprachliche Dysfunktionen wurden unter schizophrenen Patienten gefunden die auf Medikation nicht reagieren. D. H., die therapeutische Reaktion auf Medikation war der Hauptpradiktor für die Stärke der sprachlichen Dysfunktion,2. Das Gruppenprofil der sprachlichen Dysfunktion war für Reaktande und Nicht‐Reacktande in seiner Stärke jedoch nicht in seiner Form unterschiedlich,3. schizophrene Reaktande und Nicht‐Reaktande waren bei einer standardisierten Bildbeschreibungsaufgabe in ihren Leistungen vergleichbar, die oberhalb des Niveaus einer leicht‐bis‐mit‐tleren Aphasie lagen.Weitere Ergebnisse weisen darauf hin, daß nicht‐sprachliche Faktoren wie Aufmerksamkeit und logische Folgerung durch eine Beehandlung beeinflußbar sein könnten. ABSTRACTSThe Arizona Battery for Communication Disorders of Dementia (ABCD) (Bayles&Tomoeda, 1993) was given to groups of young and old normal subjects and to subjects with moderate Alzheimer's disease in the UK. Very few significant differences were found between their raw scores and those of the original standardisation sample in the USA, which makes the ABCD an appropriate test to use in the UK. Although some subjects commented on several of the test stimuli, cultural differences in pictures and vocabulary did not produce notable effects on test performance and so alterations to test materials are not required for the test to be used in the UK.La Batterie de Tests de l'Arizona pour les Troubles de le Communication (sigle anglais: ABCD) (Bayles&Tomoeda, 1993) a été administrée en Grande‐Bretagne à des sujets normaux jeunes et vieux, ainsi qu'à des sujets atteints modérément de la Maladie d'Alzheimer. L'on a trouvé très peu de différences significatives entre leurs niveaux bruts de performances et ceux de l'échantillon de standardisation originel provenant des Etats‐Unis, ce qui démontre que l'ABCD peut très bien s'utiliser en Grande‐Bretagne. Meme si certains sujets ont fait des réflexions sur quelques stimuli du test, les différences culturelles concernant les images et le vocabulaire n'ont pas produit d'effets notables sur les résultats, de sorte que l'on n'a pas besoin de modifier les matériaux du test pour l'utiliser en Grande‐Bretagne.The Arizona Battery for Communication Disorders of Dementia (ABCD) wurde in Großbritannien bei Gruppen von jungen und alten Normalpersonen und bei Personen mit mittelschwerer Alzheimererkrankung angewandt. Sehr wenige signifikante Unterschiede wurden zwischen diesen Rohdaten und denen der ursprünglichen Standardisierungsstichproben in den USA gefunden. Dieses Ergebnis bestätigt, daß die ABCD ein für Großbritannien geeigneter Test ist. Während einige Personen Bemerkungen über mehrere Teststimuli machten, hatten kulturelle Unterschiede in den‐Bildern und im Wortschatz keine bemerkenswerten Folgen für die Testleistung. ABSTRACTSThis paper reports the pilot of a new technique for the development of students' clinical skills. A bank of simulated clients was used in the teaching of generic clinical skills to two groups of speech and language therapy students. The method for the training of actors and running of sessions is described in detail by use of both group and individual formats. The intervention was used with three groups of students at different points in a 2‐year Masters degree course. Questionnaires to students evaluate the technique and the timing of its use in the course. It is concluded that this new type of clinical teaching is a valid and much‐needed supplement to traditional individual National Health Service (NHS) placements.Cet article rend compte d'une nouvelle technique pour développer les capacités cliniques des étudiants. Un groupe de pseudo‐clients est utilisé pour enseigner les capacités cliniques génériques à deux groupes d'étudiants en orthophonie. L'article décrit en détail la méthode d'entraînement des acteurs et le déroulement des sessions avec les étudiants au moyen de formats individuels et de groupes. L'intervention a été utilisée pour trois groupes d'étudiants à différents moments de leurs deux années de cours de Maîtrise. Les questionaires remplis par les étudiants en évaluent la technique et l'organisation temporelle lors des cours. L'on conclut que ce nouveau type d'enseignement clinique constitue un supplément valable et très utile aux placements individuels traditionnels dans les services médicaux.Dieser Aufsatz berichtet vom Pilotversuch einer neuen Methode zur Entwicklung klinischer Fähigkeiten bei Studenten. Eine Datenbank simulierter Patienten bzw. Klienten wird für das Lehren grundlegender klinischer Fähigkeiten bei zwei Gruppen von Studenten der Sprachtherapie verwendet. Die Methoden für das Schauspielertraining und für die Führung von Behandlungen werden unter Anwendung sowohl des Gruppen‐ als auch des Individualformats im Detail beschrieben. Die Methode ist bei drei Studentengruppen zu verschiedenen Zeitpunkten während eines zweijährigen Postgraduiertenkurses eingesetzt worden. Fragebögen von den Studierenden beiten eine Evaluierung der Methode und der Zeitpunkte ihrer Verwendung im Kurs. ABSTRACTSA vocal health questionnaire was administered in three locations in the USA to female university singers and non‐singers. The responses of 70 singers and 89 non‐singers were analysed. The questionnaire solicited background information as well as information about vocal health and symptoms, vocally abusive speech habits, attitudes toward singing and vocal health, feelings about the voice, and general affective/dysphoric tendencies. There were marked, sometimes diametric, differences between singers' and non‐singers' responses, attesting to the unique attitude and heightened sensivity and concern of singers regarding voice, singing and vocal health.Un questionnaire sur l'état de leur Système vocal a été administré dans trois sites universitaires américains à des femmes universitaires, dont certaines chantaient et d'autres ne chantaient pas. L'on a analysé les réponses de 70 des premières et 89 des secondes. Les questions portaient sur leurs antécédents, et également sur l'état de leur voix, leurs symptômes vocaux, leurs habitudes néfastes pour leur voix, leurs attitudes envers le chant et la santé vocale, leurs sentiments à propos de la voix, et leurs tendances générates affectives/dysphoriques. Des différences marquées, parfois diamétralement opposées sont apparues entre les réponses de celles qui chantaient et celles qui ne chantaient pas, ce qui atteste de l'attitude particulière des premières, et de leur sensibilité accrue envers la voix, le chant et l'état de leurs cordes vocales.Ein Fragebogen zur Stimmhygiene wurde an drei Orten in den USA einerseits an Gesangsstudentinnen und andererseits an Studentinnen anderer Fächer verteilt.
