Many families, when presented with the option of family therapy, are less than eager to participate. This paper comprises the second and third parts of a series on engaging “resistant” families.1Part II presents 21 principles and a number of techniques and strategies that have been developed for successfuly recruiting such families. These techniques should be applicable for engaging resistant families with all types of presenting problems.Part III provides an analysis of the important variables involved, along with data on cost efficiency and administrative costs. It was found that when therapists had administrative control of their cases, serving in dual roles as both therapists and drug counselors, the recruitment effort was (a) more effective (i.e., complete families, including both parents or parent surrogates, were recruited in 77 per cent of the cases), and (b) twice as cost efficient. Two‐thirds of the non‐engaged families were not recruited because the index patient would not allow family members to be contacted. Black families were more difficult to recruit than whites. Data on cost efficiency and on the actual administrative costs of recruiting families are also provided.We conclude that the engagement process requires a revision in therapeutic philosophy, since such families are often desperately in need of help but are unavailable unless therapists make a special effort to rea

This paper presents an analysis of sibling roles in families that have problems related to adolescent chemical abuse. It is based on material gathered during an exploratory research project directed toward structural evaluation of these families. The analysis identifies even sibling roles. The roles of Parental Child, Good Child, and Symptomatic Child are characterized as those with major structural impact. Four auxiliary roles of child Advocate, Analyst, Peacemaker, and Therapist are also described. Structural implications of sibling roles are discussed and suggestions for clinical intervention are presented.

Various procedures for training parents to be play therapists for their own young children are described. In their play encounters with the child, not only is each of the parents learning ways of being with the child that are indicative of empathy and caring, but they are also learning that the way each acts toward the child in the playroom (and in the home) is reflective of their own conflicts as persons and as spouses.

The relationships between sex and birth order of the sibling set, number of parents in the primary family unit, and family dysfunction were assessed. The sample was restricted to two‐child and three‐child families, all of school age, and all living with one or both natural parents only, i.e., no one else was in the household. Chi‐square analyses indicated that the likelihood of a family seeking treatment for one of their children was lowest with all girl sibling sets, intermediate with all boy sibling sets and with sibling sets in which all the girls were older than all the boys, and highest with sibling sets in which at least one boy was older than one girl. The patterns were stronger for two‐parent than single‐parent families. The results were interpreted in terms of two factors: (a) the relative commitment of boys and girls to supporting family norms, and (b) the relative stability of a family culture in same gender versus mixed gender sib

Parents of schizophrenics (N = 17) were compared with parents of nonschizophrenic hospitalized patients (N = 14) and parents of normals (N = 15) on the degree of symbiotic enmeshment and confirmation of individuality in the parental relationship. Symbiosis was divided into six component variables: undifferentiation, dependency, intervention, disapproval of other relationships, separation difficulty, and injunctions. Confirmation was bifurcated into acknowledgment and accommodation. Projective tests (TAT and Make‐A‐Picture‐Story) and interviews were used to measure both variables. The results suggest that mothers of schizophrenics tend to be symbiotically attached to their husbands and view their husbands as forming the same type of attachment to them. J.L. Moreno's fundamental and pioneering contributions to the development of group therapy, encounter, transactional analysis, and especially psychodrama are well known. However, most family therapists seem unacquainted with the fact that from his earliest writings in 1923 J.L. Moreno developed an interactional view of psychotherapy that in 1973 already resulted in formulations of a true systems orientation and very concrete ideas about marital therapy, family therapy, and network therapy. He probably is the first (1937!) therapist that actually involved a husband's lover in conjoint marital therapy. His general theoretical formulations about the pathology of interpersonal relations as well as his practical suggestions for the therapy of the interpersonal relations seem to be insufficiently known to workers and researchers in the field of family therapy.

Measures of parental affective style of communication, based on direct family discussions, were obtained for a sample of 52 families of disturbed but nonpsychotic adolescents. When the variations of both parents on the measure for two different measurement contexts were combined, an accurate prediction of subsequent psychiatric status at follow‐up occurred. Adolescents with at least one parent who consistently displayed a pathological affective style of communication in both a dyadic and triadic discussion developed schizophrenia‐spectrum disorders as young adults. Conversely, adolescents with at least one parent who was consistently benign in affective style had healthier outcomes. Finally, more serious young adult psychopathology was noted where both parents were inconsistent in their affective style from dyadic to triadic interaction. This paper discusses the need for the development of new research designs for family therapy evaluation, and a manner for meeting that need. The single‐case research design has gained acceptance as bona fide experimental designs for evaluating the effectiveness of intervention techniques in drug and psychotherapy outcome research. Although the need for more outcome studies on the effectiveness of family therapy has been frequently noted, there has been virtually no use of the single‐case design in family therapy outside of the behavior modification literature on families.
