This article describes the process of an evolving research project. Initially conceived as a study investigating outcome measures and their sensitivity to change after a course of family therapy, the project soon changed its focus. As unexpected results were recorded, the clinical research team became destabilized and the individual team members responded by making their own “sense” of the data, reflecting their respective clinical and scientific positions. As clinicians and researchers began to challenge each other's belief systems, the project entered a new stage. The interactions within the team became of increasing interest and themselves objects of research. The recursive nature of re‐search was demonstrated, and the act of writing this report completed the circle, as the various authors tried to achieve a balance between reporting the content and the process of this pr

The diversity of mental health care “schools” or therapeutic perspectives poses problems in selecting and using any single therapeutic perspective. This article describes the genesis of clinical controversy—that is, how two or more therapeutic perspectives of the same clinical situation can be so different yet not necessarily differ in outcome. Ideas drawn from contemporary philosophy of science show how different “schools” derive separate, incompatible sets of scientific evidence from the same clinical situation. The school or theory determines (in part) not just what evidence is used, but what evidence is actually perceived. The authors conclude by recommending a pluralistic approach to mental health care; they map out some consequences of this pluralism and suggest some strategies for minimizing the disadvantages of “mixing and matching” therapeutic

The complete adoption of a narrative paradigm for therapy is proposed and its implications and possibilities spelled out, especially for life in a post‐modern world that lacks any objective frame of reference. In the post‐modern sensibility, the story is set free to perform as simply a story that allows for re‐invention as the story‐teller finds a voice rooted in the person's own experience and in the connection of her story to those of others, and to larger stories of culture and humanity. The realization that we are all characters in each other's stories as well as our own reminds us that our stories only go forward as we act in ways that also forward the stories of others. A hermeneutics for such therapy validates and questions all points of view so that they do not harden into stories that are assumed to be objective

Previous evaluations of the Circumplex Model's curvilinear hypothesis using FACES instruments have yielded conflicting results. A review of the different research procedures and samples used in those investigations revealed that none of the studies had samples large and/or heterogenous enough to test the curvilinear hypothesis adequately. The present study evaluates the curvilinear hypothesis of family functioning and the concurrent validity of FACES III with a sample of optimal size (N = 2,440 families) and diversity. The lack of support for the curvilinear hypothesis in this “greenhouse” sample is explained by different findings for the two FACES III subscales. There was no relationship between the study's measures of well‐being and the adaptability subscale and a linear relationship between these measures and the cohesion subscale. In a randomized clinical trial of Inpatient Family Intervention (IFI) for 169 inpatients with schizophrenia, affective disorder, and a residual group of other diagnoses, results suggested significant effects favoring IFI for patients and their families. The treatment effects were limited to females and to two diagnostic groups: chronic schizophrenia patients and the bipolar subgroup of affective disorders. Using analysis of variance and correlational analyses, we examined three variables that mediate between family treatment and outcome: 1) posthospital medication compliance, 2) posthospital psychosocial treatment compliance, and 3) patient rejection by the family. Results showed robust correlations between these variables and outcomes for all diagnostic subgroups. The pattern of associations between the mediating variables and outcome was more prominent for patients treated with IFI. These results were seen most clearly in the total sample, and for the subgroups of “all females” and “poor prehospital functioning females with schizophrenia.” Finally, family achievement of the goals of the family intervention were positively associated with better patient outcome, while increased family burden at discharge was associated with shorter length of time posthospital to rehospital

This article presents a new theory that separates the levels of communication and relates them circularly, namely, by separating time from space/meaning variables. Documenting this proposition requires sequential microdescriptions—a far‐out project in the field of family therapy. In an extensive study of clinical and nonclinical families, starting with available microana‐lytic data on nonverbal parent‐infant dialogue, distinct time organizations have been found to modify the degree of circularity between the levels of interaction according to the observed types of engagement, that is, consensual, conflictual, and paradoxical. The double description of the dyad as a totality versus the dyad as a framing/ developing organization imparts crucial information on how development proceeds in dyadic, co‐evolutive systems, and presumably in larger ones too. Meaningful cross‐cultural application of family therapy theories requires that basic assumptions about normal family functioning be made explicit, and contextual factors be incorporated into the theories. Guided by this understanding, this article examines the family structure dimensions of proximity and generational hierarchy theoretically and empirically. These dimensions and their superordinate construct, boundary permeability, are analyzed in terms of their underlying assumptions. The cross‐cultural validity of the dimensions is assessed using videotaped interactions, projective drawings, and interviews with a sample of 24 Turkish families. Results indicate that proximity is a valid dimension, moderated by the demographic variables of socioeconomic status (SES), mother's working status, family size, and clinical status.
