Periodontal diseases are common oral diseases that afflict all humans to some degree. The major aetiological agent is dental plaque – the complex microflora which forms on teeth in the absence of effective oral hygiene. The interaction of the microbial flora and the periodontal tissues produces an inflammatory response and tissue breakdown. Recent information has categorized periodontal diseases on the basis of increased knowledge about the particular microorganisms associated with the different clinical conditions. In addition, the important role of host defences, in particular the phagocytic cellular elements, has allowed for a better understanding of the pathological processes. This knowledge is contributing towards the development of rational and effective therapy for all forms of periodontal diseases. Because of the widespread occurrence of periodontal diseases and their potential relationships to systemic conditions, it is important that medical practitioners should be able to recognize, and be conversant with methods of treatment of, these diseases.

The use of amiodarone, a drug which is prescribed increasingly as an anti‐anginal and anti‐arrhythmic agent, necessitates a high index of suspicion for the development of thyroid disorders, especially thyrotoxicosis. Two cases, which illustrate the diagnostic dilemma of hyperthyroxinaemia and the poor response to antithyroid medication, are described. During amiodarone therapy, the clinical features of thyrotoxicosis may be masked or atypical, and the choice of therapy is complicated by a delayed response to thioamide drugs and possible contraindication for beta‐blocking agents which necessitates the use of glucocorticoid drugs in some patients.

Nearly 50% of the general practitioners in nine representative local‐government areas of Sydney spoke one of 27 languages other than English. The most common languages were Chinese dialects, Indian dialects, Italian, German and French. Bilingual and multilingual doctors tended to practise in local government areas with relatively large non‐English‐speaking populations. Nevertheless, some language groups would not have easy access to a general practitioner, particularly to a female doctor, who speaks their language.

A self‐reported survey of 878 medical students in Monash University (excluding students sponsored by overseas governments) showed that 21.4% had a fluent command of a language other than English, and 62.2% had a basic usable knowledge of another language. Only 28.5% of the students reported having neither a fluent nor a usable command of another language. These results suggest a rich resource of skills in community languages other than English among people who will be entering the medical workforce. Positive recommendations are made to develop and use this resource effectively.

To determine whether asthma can cause irreversible airflow obstruction (IAO) 89 subjects with uncomplicated asthma received intensive treatment for four weeks. FEV1was measured at 0, two and four weeks; pulmonary elasticity and flow resistance was measured in 46 of the subjects at four weeks. Severity of asthma and cigarette consumption were ascertained by a questionnaire. The mean difference between the predicted and highest FEV1during treatment was 0.29 L (P<0.001). The highest FEV1% predicted correlated (P<0.001) with the duration and severity of asthma in the entire group, in the 51 non‐smokers, and in the 47 subjects with the adult onset of asthma. Pulmonary resistance, but not elastic recoil, correlated with the duration and severity of asthma (P<0.01). The results show that chronic asthma can cause narrowed airways and IAO, and suggest that this may be prevented by improved control of asthma.
