An 11‐year review is presented of 73 patients treated for gas gangrene at The Prince Henry Hospital. The geographical collection area extended throughout the States of New South Wales and Queensland and to Papua New Guinea. The commonest aetiological factor was found to be motor‐vehicle trauma, motor‐cycle accidents producing compound limb fractures in particular. The infecting organisms in these cases were most commonly clostridia. The diagnosis of the disease remains a clinical one. Treatment consisted of three facets: conservative surgery, as much of the limb or viable tissue as possible being preserved; high‐dose penicillin administration; and hyperbaric oxygen therapy. Death occurred in 15 cases, gas gangrene being the principal cause in seven. However the incidence of gas gangrene has diminished since 1971, and there are hopes that it will decrease further through adequate prophylaxis and prevention in civilian surgical practice.

The results are reported of the intrauterine diagnosis of fetal thalassaemia in 72 pregnant women investigated during the six years from 1977 to 1982. In the majority of cases fetal blood was collected by fetoscopy for globin chain synthesis at 18 to 19 weeks’ gestation. Adequate fetal blood samples for the diagnosis of the thalassaemia status of the fetus were obtained in 85% of cases. The technical difficulties associated with fetoscopy and fetal blood collection are discussed, and the maternal and fetal complications of the procedure described.

During March, 1981, a number of cases of dengue fever occurred in Cairns and Townsville, northern Queensland. From October, 1981, an outbreak of the infection was recognized on Thursday Island and, by May 1982, an estimated 38% of residents had been infected. Isolated cases were reported from other towns in northern Queensland and from other islands in the Torres Strait. Clinical presentation varied from that of severe incapacitating illness lasting up to seven days to infections which were confirmed by serological tests, but were not associated with apparent illness. No deaths were reported. Entomological surveys indicated that the domestic breeding vector of dengue, Aedes aegypti, is widely distributed throughout Queensland – southwards to Dirranbandi and westwards to Mornington Island. In some localities, the indices of Ae. aegypti abundance are alarmingly high, but at least in some suburbs of Townsville, it has been effectively controlled.

Seventy prostitutes were screened at their place of work, a Sydney house of prostitution, on a weekly basis over one year. Of these, 10% acquired new infections with gonorrhoea each week (53 episodes). Clinical guidelines (symptoms, contact history, physical signs) were found to be unreliable, in this context, for predicting the isolation of Neisseria gonorrhoeae. Of the 39 women observed over one month or more, 17 (44%) acquired gonorrhoea within the first month. Stability of the place of work appeared to be associated with a lower isolation rate (5.5%). Five asymptomatic, urethrally infected men (two clients, three boyfriends/husbands) were detected, and appeared to have an important role in the hyperendemicity of gonorrhoea in this environment.

All the women working in a Sydney house of prostitution were surveyed for one year for the presence of sexually transmitted diseases (STDs) and other medical problems, and for the contraceptive methods they used. Considerable potential morbidity from STD was noted, particularly pelvic inflammatory disease (23 episodes). Contraception was an area of unsatisfactory practice. Suggestions are made for the clinical management of these women, with the intention of diminishing the impact of STDs on them and, as a direct result, rendering them less infectious, to the benefit of the community. It is hoped that greater knowledge will improve the standard of current debate about prostitution.

The five‐year survival rate of patients with locally invasive bladder cancer (stages T3‐T4, Nx, Mo) is less than 50% whatever treatment is given. In an effort to improve the prognosis of patients with this disease, we have incorporated into a management protocol two cycles of intravenous administration of cis‐platinum before surgery or radiotherapy (which function as the “definitive” treatment). In 10 of the first 12 patients treated, tumour necrosis has been demonstrated histologically, and definite clinical responses have been observed in eight patients after two cycles of cis‐platinum treatment. The toxicity has been acceptable, and subsequent management has not been compromised.

The effect of cimetidine on the elimination of theophylline was studied in six patients with chronic obstructive airways disease, who were receiving maintenance therapy with theophylline. The administration of cimetidine resulted in a decrease of 25% in the interdose clearance of theophylline, which led to an increase in “steady‐state” theophylline levels in five of the patients. Prescribes should be aware of this potential interaction and reduce theophylline dosage where appropriate.
