Sera from Australian homosexual men with idiopathic lymphadenopathy syndrome (ILS), a disease related to the acquired immune deficiency syndrome (AIDS), were tested for antibodies to two serotypes of human T leukaemia virus (HTLV), types I and III; type III is the type recently recognized in the USA as being associated with AIDS. Sera were tested also from other homosexual men and heterosexual controls. Antibody to high titre to HTLV‐III was present in the serum of all six subjects with ILS. In contrast, antibody to low titre was present in only one of six healthy homosexual men and in one of five homosexual men with immunodeficiency but no lymphadenopathy. Antibody to HTLV‐III was not detected in serum in the one subject with AIDS who was tested, but was detected in his healthy sexual partner. Antibody to HTLV‐I, earlier believed to be related to AIDS, was lacking In all subjects. These results suggest, in an Australian setting, that infection with HTLV‐III is the cause of ILS and, by inference, may also be the cause of AIDS. We suggest that there are as yet undetermined factors associated with male homosexual practices which cause immunodeficiency which, in turn, increases susceptibility to HTLV‐III, the actual causal agent of ILS and, probably, AIDS.

Over a 10‐year period from December 1972, the reversal of routine surgical sterilizations in 252 women resulted in a high rate of subsequent full‐term pregnancies. The ages of these women ranged from 22 to 40 years (average, 32.5 years), and the parity ranged from 0 to 4 (average, 2.1 children). A meticulous microsurgical approach was employed for all tubal reconstructive operations. Pregnancy usually occurred within 12 months of the reconstruction (75%), but, occasionally, it occurred within the first two months (8%). In the group of 68 women, in whom the previous sterilization procedure had affected only the isthmial section of the fallopian tube, a remarkable full‐term pregnancy rate of 93% was achieved. It is, therefore, recommended that all sterilization procedures in women should be performed in the mid‐isthmic portion of the tube, because reversals of sterilizations in this area have the best chance of success.

The fungusHistoplasma capsulatum, although commonly found in bat‐frequented caves in many countries, has not previously been isolated from that environment in Australia. This report describes the isolation ofH. capsulatumfrom several sources, including cave soil, the organs of mice exposed to the cave environment, and the sputum of a patient clinically diagnosed as having acute pulmonary histoplasmosis after exposure to the cave environment.

Fifty patients with advanced, measurable, squamous‐cell cancers of the head and neck received treatment with a combination of cisplatin, vinblastine, and bleomycin (PVB). The tumours in 25 patients (50%) responded to treatment (partially in 17 and completely in eight). The median duration of response was 18 weeks, and that of survival was 31 weeks. Toxicity was moderately severe; myelotoxicity occurred in 13 patients, and nausea and vomiting in 22 patients.

Adequate disaster management depends on the incorporation of experience and research findings into future disaster plans. To assist in this process, a series of psychiatric patients examined after the Ash Wednesday bushfires in South Australia are described. The level of handicap experienced was often substantial and interfered with these people's attempts to overcome the losses they experienced in the disaster. Some people had to go to considerable lengths to seek psychiatric help because their general practitioners and bushfire relief workers did not understand the quality of their symptoms and had not arranged referral for them. The types of disorder, the time of presentation and the role played by the disaster in the onset of these disorders are described. The need for an educational and consultative psychiatric service for general practitioners and welfare workers who have contact with disaster victims is discussed.

The Ash Wednesday bushfires were one of the worst natural disasters in Australia's history. This paper documents observations of human reactions to the disaster started Immediately after the bushfires. Two frameworks were used for classification of observations: blopsychosoclal and temporal. Biological, psychological and social reactions in adults and children are described as they occurred before, during, Immediately after, and two months after the disaster. Some reactions in animals are noted. Victims of the bushfires were seen to have reacted during the various phases of the experience In a predictable way: during the acute danger, when survival was paramount, and Immediately afterward, the usual patterns of hierarchical structure within families and in the wider community broke down and new social structures emerged. These reverted to previously existing patterns In the ensuing months. Feelings of disorientation, unreality and anger at outside agencies, which were viewed as hostile and unhelpful, were commonplace. The findings have Implications for the development of everyday stress reactions and clinical syndromes presenting to clinical practitioners. Lastly, the role of intervention for disaster reactions is examined and its usefulness noted.
