Twelve patients with documented, progressive hairy‐cell leukaemia were treated with human lymphoblastoid interferon‐α. All patients initially received a three‐month course of interferon‐α(3 × 106U each day) which resulted in two complete remissions and 10 partial remissions. All eight patients who were dependent upon packed red‐cell transfusions became independent of them and the three patients with the most severe, pretreatment pancytopenia also became independent of platelet transfusions. The histological appearance of the bone marrow improved in all patients; in three patients, a complete resolution of the leukaemic infiltrate was recorded. Adverse reactions occurred in 10 patients, but these were mild and did not interrupt treatment in seven patients. Moderate reactions that required a temporary reduction in the dose of interferon‐αwere seen in three patients. Ten patients subsequently received maintenance therapy with interferon‐α(3 × 106U, three times a week). A haematological improvement continued to be seen in all patients, and the results, with a minimum of one year of follow‐up, are presented.

One hundred and sixty‐five women (primiparous, 31%) who planned to have a home birth under the care of one of six registered midwives in the south‐west of Western Australia between 1983 and 1986 were studied retrospectively. One neonatal death occurred, which was due to congenital anomalies. Sixteen per cent of the women were transferred to a hospital for the delivery. No woman's labour was induced, and the rates for assisted delivery and caesarean section were very low (5% and 1% of deliveries, respectively). The conclusion is drawn that women who choose to have a home birth in the south‐west of Western Australia may do so with a high degree of safety.

Cytological examination of the aspirate of fine‐needle biopsy is becoming more widely accepted as a screening test for malignancy in thyroid nodules. However, it tends to be used in addition to, rather than instead of, more traditional methods. In this five‐year prospective evaluation we performed fine‐needle biopsy in 618 euthyroid patients with nodular thyroid enlargement, 86% of whom also underwent radionuclide scans and 55% of whom underwent ultrasound scans. In 19% of patients fine‐needle biopsy yielded insufficient material for diagnosis, and in 14% of patients normal follicular cells were found (which indicated that the clinical lesion was not sampled). To date, a histological diagnosis has been obtained in 258 (42%) patients, 44 of whom had malignancies. The results of the radionuclide and ultrasound scans did not alter the odds in favour of the detection of malignancy. The cytological diagnosis of malignancy was falsely‐positive in two patients and falsely‐negative in four patients (three cases of which probably were sampling errors). If, in addition to overtly‐malignant cells, atypical Hürthle cells and follicular neoplasms were considered to be potentially malignant, fine‐needle biopsy alone had a sensitivity of 87% and a specificity of 72%. This good accuracy would be reduced by sampling failures, but a policy of operating on all patients with potentially‐malignant cells, or on those in whom satisfactory aspirates could not be obtained, would yield high rates of the diagnosis of malignancy and would reduce the number of operations. Our data indicate that the most‐appropriate method of screening thyroid nodules for malignancy is fine‐needle biopsy without pertechnetate scanning or ultrasound examinations.

Male sexual partners of a cohort of women with genital‐tract abnormalities which were associated with human papillomavirus infection were examined for evidence of infection with human papillomavirus. Of the 214 male partners who were examined, 93.5% had visible genital lesions. Of the 196 lesions that were biopsied, 72.5% showed histological evidence of infection with human papillomavirus, and only 20.4% of subjects with histological evidence of human papillomavirus were aware of a lesion. An unexpectedly high proportion (6.1%) of lesions on which a biopsy was performed, particularly those with flat, red, indurated morphology, also showed histological evidence of penile intraepithelial neoplasia. This was not significantly more common among the partners of the women with cervical intraepithelial neoplasia than it was among the partners of the women with other evidence of genital human papillomavirus infection. Penile intraepithelial neoplasia was significantly (P<0.001 more common among subjects with no history of non‐genital warts. We conclude that the male partners of women with human papillomavirus‐associated lesions are very likely to be infected with human papillomavirus, and thus may act as a significant reservoir for the reinfection of their female partners. As the awareness of human papillomavirus‐associated lesions was low among the male partners, colposcopic examination and treatment of their male partners, and/or barrier contraception, may be a necessary part of the management of women who are undergoing treatment for human papillomavirus‐associated genital disease.
