AbstractThe case history of a child with acute promyelocytic leukemia (APL) is reported to illustrate both an unusual presentation of APL as a pelvic mass and to review the pathophysiology and treatment of the disease. Therapy of APL consists of chemotherapy, namely adriamycin/daunomycin for remission induction, and of control of disseminated intravascular coagulation. A chloroma, if present, may require local irradiation in addition to chemotherapy. With aggressive management, the number of prolonged remissions may be greater for APL than for any other form of acute myelogenous leukemia (AML), with significant numbers of patients achieving five‐year surviva

AbstractFour children presenting with unresectable hepatoblastomas and one with metastatic disease are reported. Following initial biopsy all were treated with chemotherapy which included Adriamycin. Three of the four children showed a significant reduction in tumor size, and in two, delayed resection of the primary lesion was possible. Chemotherapy including Adriamycin represents effective initial cytoreductive therapy for childhood hepatoblastoma, thereby reducing the morbidity and mortality associated with the extensive hepatic resection usually required for an untreated lesion.

AbstractCCNU was used in the treatment of 18 patients with childhood tumors of the CNS. Severe bone‐marrow toxicity was noted. The initial dose was 130 mg/m2given every 6 weeks, but it was often necessary to reduce the dose and prolong the interval between doses. The toxicity appeared to be much the same whether previous radiotherapy had been given to the whole CNS or to the brain only.In assessable patients with pure medulloblastoma, three out of three showed marked responses to CCNU. In those with a mixture of medulloblastoma and other tumor components, two out of two showed responses to CCNU. In children with astrocytomata of various types, only two out of six showed responses to CCNU, and in these the degree of response was slight. The results suggest that CCNU is a useful treatment for medulloblastoma but its value in the treatment of childhood astrocytoma is uncertain. AbstractThe results of a chemotherapy regimen utilizing adriamycin in combination with vincristine, prednisone plus or minus cyclophosphamide (CHOP‐HOP) for the treatment of non‐Hodgkin lymphoma are compared to those of a non‐adriamycin containing combination (COP). The complete remission (CR) rate of 67% for CHOP‐HOP in malignant lymphoma, diffuse histiocytic type (MLDH) is superior to 33% obtained with COP. In malignant lymphoma, nodular poorly differentiated lymphocytic (MLNPD), the CR rate was: CHOP‐HOP 83%, COP 44%. The prognostic factors for both treatment groups were compared and found to be similar. Thus, adriamycin appears to be responsible for this improvement in CR rate. When analyzed as a group, the patients treated with CHOP‐HOP had a longer survival than those treated with COP. This difference was also evident in the subgroup of patients with MLDH. The smaller subgroup of patients with MLNPD treated with CHOP‐HOP did not show a significant improvement in survival. A trend for longer duration of CR in both diffuse and nodular lymphomas was also observed, but this difference did not reach statistical significance possibly due to the smaller numbers of patients. AbstractUrinary catecholamine catabolites, homovanillic acid, vanillylmandelic acid, total metanephrines, and total catecholamines were assayed in 12 patients with newly diagnosed retinoblastoma prior to surgery and treatment. These values were within normal limits for the ages of all 12 patients, including one with metastases to brain, liver, lymph nodes, and bone marrow. The predictive value and efficiency of these tests in the diagnosis of retinoblastoma are insufficient to warrant their continued use on a routine basis.
