摘要:
In normal conditions, there is a precise link between regional metabolism and blood flow in the brain. In pathological states there may be an uncoupling between CBF and metabolism, due to many mechanisms. In particular, anaerobic glycolisis, membrane depolarization, and altered neurotransmitter release are relevant for the final damage of the tissue. Postischemic recovery was analyzed in experiments with 14C-deoxyglucose. In men, the noninvasive approach with 15O by inhalation allowed the determination of regional oxygen utilization. In TIAs, there is a chronic ischemic condition, while in strokes there is often a relative luxury perfusion.

摘要:
A report of 12 patients with recurrent multiple cranial neuropathy is presented. In most cases the palsies accompanied a mild respiratory infection. Both motor and sensory cranial nerves were affected, the facial nerve and optic nerve being the most common. The symptoms were self-limited in course and steroid therapy seemed to hasten the recovery in the cases it was used. Only in a few cases a noticeable neurological deficiency remained. Aetiological aspects of the syndrome are discussed.

Out of 1,120 cases with focal cerebrovascular lesions, 102 cases (9.1%) were classified as transient episodes under the heading of reversible ischemic attacks (RIA). RIA comprises classical transient ischemic attacks (TIA), clearing completely within 24 h, usually few minutes or hours, and strokes in which a full recovery takes place over an average of 3 weeks. The clinical definition of transient and/or reversible requires a complete negative neurological examination including the normalization of the EEG and brain scan as well as a normal computed tomography. Even multiple transient episodes quite indistinguishable from classical TIA can be brought about by cerebral tumors. TIA are important symptoms, not a disease in themselves. Hypertension and cardiac disease along with carotid stenosis and/or occlusion seem to constitute the main conditions responsible for an evolution from TIA to completed strokes.

摘要:
The physiopathological role of platelet aggregation in some thromboembolic and atherosclerotic complications is strongly suggested on the basis of many indirect findings. The qualitative methodological approach to this problem generally used until recently is rapidly giving way to a quantitative, biochemical approach. Platelet aggregation, however, even if expressed in terms of nanomoles of a product obtained in a sophisticated reaction system, will continue to deceive investigators and clinicians who fail to view it in the adequate (although still uncertain) context of rheological and vascular interactions.

10 patients suffering from intracranial aneurysm causing subarachnoid hemorrhage (SAH) have been treated during the acute pre- and postoperatory phase with e-aminocaproic acid. The blood and the cerebrospinal fluid of the treated patients have been analyzed in order to study fibrin(ogen) degradation products (FDP) and fibrinolytic activity (FA). The results so obtained show only an alteration of local fibrinolytic processes either inside or around the aneurysmatic clot: there were no alterations of the systemic FA. Furthermore, monitoring of FDP and FA was a very useful tool in those patients who took advantage of the treatment with e-aminocaproic acid which protected them from rebleeding.

On the basis of the assumption of an affinity between cerebral and retinal vessels, it was sought to demonstrate that the variations induced in the retinal circulation are akin to those occurring at the cerebral level. A case is reported of a young girl who developed benign intracranial hypertension, with severe bilateral papilledema and a left sixth cranial nerve palsy, after minocycline therapy. This drug is a semisynthetic tetracycline which has proven to pass into the CSF more effectively and to have a greater lipoid solubility than the other antibiotics of the same group.

摘要:
The effect of piracetam, a cyclical derivative of GABA, was compared with that of a placebo in a double-blind study of 60 patients with post-concussional syndrome of 2–12 months’ duration. The daily dose of piracetam was 4,800 mg. After 8 weeks of treatment piracetam significantly reduced the occurrence and severity of the following symptoms: vertigo, headache, tiredness, decreased alertness, increased sweating and neurasthenic symptoms. No significant effect was observed on the following symptoms: tremor, orthostatic symptoms, and memory disorders. Side effects were reported by 64% of the patients under piracetam and by 32% under placebo.
