A case of CNS listeriosis in a 49-year-old man, previously in good health, is described. The illness showed a biphasic clinical pattern with a prodromal phase characterized by headache, fever and leukocytosis and subsequent development of pontomedullary cranial nerve paresis. The infection was rapidly progressive and fatal. Gram-positive rods, identified as Listeria monocytogenes, were detected in CSF samples only at the end stage of illness. Neuropathological abnormalities consisted of rhombcncephalitis and basal meningitis. The major difficulties encountered in the early diagnosis of CNS listeriosis are discussed.

Increased intracranial pressure developed in a mentally and physically normal patient following ingestion of 30 glasses of water as a voluntary preparation for ultrasonic gynecological examination. Hyponatremia resulted and induced brain edema which provoked increased intracranial pressure. Hyponatremia was treated by intravenous infusion of hypertonic NaCl and mannitol solutions. Corticosteroids were added and continued for a week. The patient totally recovered 4 days after admission.

We review clinical virological studies in the syndrome of delayed contralateral hemiplegia following herpes zoster ophthalmicus. Virus could not be isolated from the cerebrospinal fluid (CSF) of the present case, nor was antiviral antibody found in the CSF. There appear to have been no reports of successful virus isolation from the CSF although there are reports of antibody in the spinal fluid. Thus the evidence for ongoing viral replication in the central nervous system is marginal. It is suggested that the sensitive antibody assay against membrane antigens (FAMA) be used in the future as a guide to antiviral therapy.

A 53-year-old patient with adrenoleukomyeloneuropathy, showing symptoms of mental deterioration, cerebellar ataxia, spastic tetraparesis, and polyneuropathy, was reported. The myelinated fiber densities were decreased in the anterior spinal root and sciatic nerve. Most of the myelinated fibers showed segmental demyelination and a few fibers showed axonal degeneration. Electron microscopy showed numerous lipid inclusions in the cytoplasm of the Schwann cells. The muscles showed neurogenic changes. Numerous lipid bodies and intracytoplasmic crystalline arrays were found in the sarcolemma.

This study examined the validity of evoked somatosensory potentials as a measure of subclinical neurological damage to workers chronically exposed to nonorganic mercury. In this study potentials were recorded along the somatosensory pathway from the periphery to the primary cortex in response to electrical and mechanical stimuli. The findings of this study indicate that such workers exhibit subclinical damage which manifested in a delay in nerve ending conduction times at the periphery, and an acceleration of the conduction from brain stem to cortex. These findings support the suggestion that evoked potentials may be a sensitive and reliable measure in the detection of subclinical neuropathic phenomena. They may consequently be utilized as an efficient early warning system in the prevention of clinical symptoms.

73 primary epileptic patients, grouped according to their clinical picture and EEG focality, were evaluated by means of a test of tactile extinction (Quality Extinction Test) in order to assess the usefulness of this test in detecting lateralized cerebral malfunctioning in neurological functional diseases. The data indicate an increase of tactile extinction in epileptic patients and a good relationship between side of focality and side of extinction.

A new endoscopic technique is presented, which allows burr hole evacuation of intracerebral hematomas under video control. Clot removal is achieved using a suction-irrigation system built in the 6-mm-diameter endoscope; in addition, an ultrasonic morcellement device has been developed for transendoscopic application. Small bleeding vessels in the hematoma cavity can be coagulated using a 1.5 mm Neodym YAG Laser microprobe introduced through the endoscope under visual control. A randomized prospective study has been started to raise again the question of indications for the surgical treatment of intracerebral hemorrhages, comparing medical treatment with the new surgical technique.

The acute effects of intravenously administered L-acetylcarnitine (L-AC) were evaluated in 5 healthy and 20 diseased volunteers (17 vascular, 3 tumoral cerebral lesions). Short-latency scalp somatosensory-evoked potentials (SEPs) to simultaneous median, and separate unilateral peroneal nerve stimulation were carried out before and after L-AC administration (at 10-, 30- and 60-min intervals). L-AC did not influence peak and interpeak latencies; however, in a percentage of healthy and diseased volunteers a clear-cut amplitude increase was evident affecting all those peaks generated between the thalamus and the cortex.While in normal and tumoral volunteers the voltage increase was bilaterally balanced, the amplitude increments were more evident on the ‘affected’ hemisphere in vascular patients, partially reversing the abnormal amplitude ratios between homologous peaks on ‘healthy’ and ‘affected’ hemispheres. In no case were transient clinical changes, either of an objective or subjective nature, associated with SEP amplitude changes; these were still present at the 60th minute, having reached their nadir at the 30th minute in

37 procedures were carried out in 32 patients with trigeminal neuralgia who were unrelieved by anticonvulsants. Initial relief was achieved in 31 patients (98%). 5 patients (16%) suffered a relapse during a follow-up period of 15–26 months.
