The author gives a survey of the present knowledge and of modern trends in the management of the neurogenic bladder in children with spina bifida, including surgical interventions, electrical stimulation and urinary tract infections. The pharmacology of receptors seems to promise a better understanding of the physiology and pathophysiology of the bladder.

The clinical data of two patients with Refsum’s disease are presented. All the cardinal symptoms and signs were present. In addition the patients had fingernails which showed peculiar abnormalities. The dietary treatment installed to reduce the phytanic acid accumulation resulted in an unexpected severe weight reduction, rapidly increasing levels of serum phytanic acid and a severe deterioration of the clinical condition. Plasmapheresis was found to be an efficient mode of treating high levels of serum phytanic acid. The patients have been observed for nearly 4 years at changing levels of serum phytanic acid. It is our opinion that a causal relationship exists between the phytanic acid accumulation and, at least, the signs of polyneuropath

Possible mechanisms producing respiratory waves in the cerebrospinal fluid pressure (CSFP) were investigated in cats and rab- bits. Differences between spontaneous and hyperpneic breathing as well as artificial respiration in the pressure waves were observed. The effects of central inhibition produced by pentobarbital and peripheral respiratory arrest (curare) were found to differ. Central inhibition resulted in a total absence of respiratory waves in CSFP as well as in arterial and venous blood pressure recordings. On the other hand, neuromuscular block was followed by the delayed appearance of slower waves, in all pressures recorded. The appearance of these late waves could be correlated with the CO2 level in the arterial blood.

Four reticulosarcomas presenting as butterfly tumours are described. In all 4 cases, tumour was not found outside the brain. It turned out that reticulosarcomas in our series comprised nearly 1/3 of all butterfly tumours, whereas 1/3 of the reticulosarcomas of the brain showed this location. In view of the inoperability of butterfly tumours and the possible radiosensitivity of reticulosarcomas, it is urged, in a case of a butterfly tumour, to try to establish the diagnosis before any decision as to treatment is made.

The following problems were considered: (1) nature of the asynchronous response in the glabellar reflex (GR); (2) changes in the GR parameters induced by L-dopa; (3) changes induced by the drug in the masseter muscle silent period (SP); (4) meaning of the glabellar SP after synchronous and asynchronous reflex response; and (5) mode of suprasegmental control by the descending dopaminergic pathways. It is concluded that the synchronous response is probably multisynaptic, in view also of the changes induced by L-dopa in parameters of some GR components. The published data on the silent period in Parkinson’s disease were confirmed in the masseter muscle; some data on the SP after glabella tap are also reported. Lastly, it is supposed that the nigrostriatal dopaminergic pathways have two different actions over the segmental circuits of the facial reflexes: inhibitory over GR asynchronous response; facilitatory over the masseter and glabellar SP. Moreover, the facilitatory action could bias different systems of spinal inhibition over the peripheral afferences, which come from different receptorial structure

The case of a young girl with Hodgkin’s disease of 2 years duration is described. During her disease, headache, dizziness and blurred vision started. Papilledema and right homonymous hemianopsia were detected. EEG, echoencephalogram, brain scan and carotid arteriogram pointed to intracranial invasion by the disease. Therapeutic test with steroids brought to amelioration; therefore, subsequent skull irradiation and chemotherapy were given which improved her condition markedly. After a remission of 10 months, Jacksonian seizures and other neurological aberrations started. In spite of therapeutic efforts, the patient died shortly thereafter. The newer diagnostic procedures, in our opinion, enable the correct diagnosis of intracranial involvement in Hodgkin’s disease. In doubtful cases, therapeutic test with steroids, followed by chemotherapy and skull irradiation, are indicated, and surgery should be advised only if these measures f
