Among the reported complications of the antifibrinolytic therapy in the treatment of ruptured intracranial aneurysms, nausea and vomiting, cerebral arteriopathy and secondary hydrocephalus would be a matter of concern in its possible CNS involvement. However, little is known about the possibility of direct cerebral effect in synthetic antifibrinolytics. A simply designed animal experiment disclosed remarkable effects on electroencephalography, blood flow of the cerebral cortex and intracranial pressure. Such antifibrinolytics should not be given intrathecally in any situation even if the accumulation of the agents in cerebrospinal fluid is essential to prevent recurrent hemorrhage.

Cerebrospinal fluid was analyzed from 8 patients with Creutzfeldt-Jakob disease. No special abnormalities were found concerning cells, total protein content or the concentrations of albumin and IgG. Routine agar gel electrophoresis was normal. Grey and white cerebral matter were analyzed from 1 patient. The total protein content was reduced in grey matter and on isoelectric focusing a distinct band at pH 8.8 was absent.

This communication surveys 398 consecutive head injury patients who were in regular full-time employment at the time of injury and who were examined in connection with a claim for compensation. In uncomplicated cases there was a close correlation between the duration of posttraumatic amnesia and the length of time off work. Some common causes of prolonged organic disability are mentioned. The recovery rate of patients suffering from posttraumatic emotional disorders, such as depression or anxiety, was proportional to the initial severity of injury, whereas the indefinite invalidism associated with so-called ‘accident neurosis’ was found to be rare in severely injured patients. ‘Accident neurosis’ also was more common in manual workers sustaining accidents at work than in the same occupation group after road accidents, or in non-manual workers. It is concluded that posttraumatic emotional disorder and ‘accident neurosis’ are not the extremes of a spectrum but separate and distinguishable

A study was performed on the reliability of several published methods predicting doses of phenytoin that will yield desired high plasma concentrations. In 28 patients with low steady-state plasma concentrations, doses were changed stepwise until a target concentration of approximately 15–20 µg/ml was reached. Predictions about plasma concentrations according to four methods were compared with actually observed plasma concentrations. It appeared that prediction was hazardous and if applied would frequently have yielded plasma concentrations in the toxic range. However, the nomogram of Richens, using a Km of 4, was rather safe. In a review of the roentgenological files of 22 patients suffering from spastic paraplegia due to neurolathyrism, 3 patients were found to present abnormal skeletal findings. 2 patients showed absence of union of the secondary ossification centres of the iliac crests, the ischial tuberosities and the vertebral bodies. Their age at the time of ingestion of poisonous Lathyrus sativus plants was 19 and 20 years. The 3rd patient had bowing and thickening of his right femoral shaft. He was 22 years old at the time of poisoning. These findings which are similar to those found in experimental osteolathyrism have never been described in clinical neurolathyrism. They are of a nature to throw some doubt on the correctness of the currently prevailing concept of sharp distinction between these two pathological entities.

A patient with typical clinical, laboratory and biopsy findings of eosinophilic fasciitis and fulfilling criteria for myositis is reported. Muscular involvement in eosinophilic fasciitis is reviewed and relations to hypereosinophilic syndromes and rheumatoid diseases are discussed. The importance of clinical recognition of eosinophilic fasciitis and histopathological examination of the fascia in the diagnosis of myopathies are underlined.
