Some immunological parameters of patients with multiple sclerosis (MS), of patients with other neurological diseases and of healthy volunteers were compared. No significant differences between these groups of persons were found in the number of leukocytes, granulocytes or lymphocytes per milliliter of peripheral blood. A density distribution profile of the peripheral blood mononuclear cells revealed a relatively increased number of low density cells (≤ 1.069 g/cm3) and a relatively decreased number of cells with higher density (1.069–1.073 g/cm3) in MS patients as compared to healthy controls. The spontaneous activity and the pokeweed mitogen-induced activity of the different cell subfractions, separated according to density, appeared to be normal in MS patients. The results suggest a continuous stimulation of the immune system of MS patients resulting in a shift of cells towards lower density. This change was not related to disease duration or sever

We describe a case presenting continuous muscle fiber activity syndrome, which was slightly reduced by the blocking of nerve conduction and abolished by ischemia. Clinical, electromyographical and bioptical evidence of marked abnormalities of peripheral motoneurons were observed. Association with central motoneuron lesions was suspected due to the observation of initial pyramidal signs. Therapy with diphenylhydantoin and/or carbamazepine resulted only in discrete amelioration of the syndrome. This case, which presented similarities as well as differences to the cases described by Isaacs and to those reported by other authors after 1961, stimulates a critical discussion of the already raised problem of the clinical heterogeneity of continuous muscle fiber activity syndrome.

In 63 patients with sympotms of central nervous system involvement, serum anc cerebrospinal fluid (CSF) samples were used to investigate C-reactive protein (CRP) by double immunodiffusion. CRP was detected in 28 sera. It was never found in CSF. The finding, by applying this technique, of CRP in the serum only was not useful for the diagnosis of central nervous system disease. The content and fate of CRP in CSF must be approached with more sensitive methods.

A patient affected with multiple sclerosis showing flexion of the knee on elicitation of the patellar reflex was observed. Electrophysiological investigations (surface EMG recordings from quadriceps, biceps and peroneous longus, M and F responses on stimulation of the femoral nerve, H reflex on stimulation of the posterior tibial nerve) were performed. The results allowed us to localize the demyelinating plaque and indicated that there was not inversion but dissociation of multiple reflexes elicited by tapping the patellar tendon.

We planned a double-blind cross-over study of memory processes in patients with common types of amnesia after administration of vasopressin, physostigmine and placebo in order to validate the possible role of these drugs in the treatment of organic amnesia. Memory, learning, vigilance, attention and concentration were tested in 24 organic amnesic patients after the three treatments. No statistically significant differences among the treatments were found.

Cervical myelopathy due to ossification of the posterior longitudinal ligament of the spine is reported in a 48-year-old Caucasian Jewish man. Clinical and roentgenologic changes are presented. The value of computerized tomography in the diagnosis of this condition is emphasized.

An electrophysiological investigation on 29 dialysis patients was performed with the aim of verifying whether serum parathyroid hormone (PTH) and aluminum (Al) levels may play a role in the pathogenesis of uremic neuropathy. All patients except 1 showed electrophysiological abnormalities. Both PTH and Al levels were found to be significantly correlated with the time on dialysis. On the contrary, the values of each single electroneurographic parameter did not show any correlation with the duration of dialysis, PTH levels and Al levels. The authors conclude that PTH and Al does not seem to play a role in the etiology of uremic neuropathy.

Some clinical and etiological aspects of senile dementia of the Alzheimer type (SDA) were elucidated in a case control study of 63 SDA patients and 91 controls. The SDA patients had significantly more commonly both senile and presenile dementia in parents and sibs than the controls. The occurrence of senile dementia in parents or sibs increased the risk for SDA by about 900% when all other relevant factors were taken into account in multiple logistic risk function analysis. The increase of the risk for SDA associated with the occurrence of presenile dementia in sibs was even greater but not statistically significant because of other factors and sample size. There was a negative correlation between the occurrence of hypertension and angina pectoris and SDA, which may be partly due to selective diagnostic criteria of SDA. The β-globulin fraction of cerebrospinal fluid was significantly reduced in the electrophoresis in the SDA patients compared with the controls

10 cases of nonconvulsive status epilepticus following the termination of generalized convulsive seizures are reported. The stupor status occurred in generalized primary epilepsy (6 cases), in generalized secondary epilepsy (2 cases), and in partial epilepsy (2 cases). Except for generalized secondary epilepsy, the status epilepticus was successfully treated by the intravenous application of anticonvulsants. The therapeutic significance of a differentiation of postconvulsive stupor status from postictal confusional states was stressed.
