Shortly after spinal cord transection at T8 in cats immobilized with Flaxedil the electrocorticogram continuously showed synchronized activity intermingled with runs of desynchronized activity. Similar changes were seen in the mesencephalic reticular formation, thalamic reticular formation and in the nucleus ventromedialis thalami. No significant changes were seen in the nucleus reticularis pontis oralis. Electrolysis at different levels of the reticular formation did not alter the synchronized electrical cortical activity induced by the spinal cord section at T8.

Operation for a subdural haematoma in a 3½ month-old boy caused severe loss of blood, with cardiac arrest lasting for 30 min. By external heart massage a minimal residual circulation was secured. Survival was 11 months in a decerebrated state. Histological examination revealed widespread cortical necrosis due to severe hypotension and oligaemia, the cortex under the trephination hole remaining unaffected. Thus, that part of the brain which was not compressed could escape during the presumed concomitant brain swelling, remained intact, and its blood supply became relatively better. According to the authors this case supports the hypothesis that cerebral swelling may play an important role in the development of hypoxic-ischaemic states

Three clinicopathologic case reports are compared with 28 cases described in the literature. Clinical features are sudden sensory onset, rapid and complete flaccid paraplegia, sometimes ascending signs, transient highly inflammatory CSF, and continuous, rapid evolution. The anatomical findings include massive spinal transverse necrosis, usually thoracic but sometimes more extensive, the grey matter being little affected, with moderate histiocytic proliferation in the early stage but no perivascular lymphoplasmocytic reaction. On the whole, the vascular abnormalities seem secondary, and there is no obvious vascular etiology. The nosological position of this entity is imprecise, but it seems to be related to demyelinating diseases.

A study was made of the EMG in 20 normal test subjects (10 men and 10 women), 10 hemiparetic patients with a pyramidal syndrome, and 10 Parkinson patients. Under standardized conditions, a constant tension of 2,000 g was exerted with the adductor pollicis muscle during 60 sec. The EMG of this muscle was derived with surface and needle electrodes, submitted to analogous digital conversion and analysed with the aid of a PDP 8/I computer. Special attention was focused on the study of change in integrated amplitude, peak-to-peak amplitude, and the number of peaks in response to fatigue. It was found that women in the normal group had a lower ‘efficiency of electrical activity’ than normal men. In paretic patients, amplitudes measured with surface electrodes were relatively high and those measured with needle electrodes relatively low (poor synergistic action). The normal increase in amplitude in response to fatigue is absent in measuring with needle electrodes, but is observed with surface electrodes (exhaustion compensated by adjacent muscles of the same group). Two familial cases of a particular form of congenital myopathy are reported. The peculiar morphological features of the muscle included hypotrophy of type I fibres with central nuclei, and central myofibrillar lysis predominantly affecting type II fibres. Electromyographic findings demonstrated the involvement of tonic motor units. Biochemical investigations suggest an arrest in the maturation of muscle cells, induced by denervative interference of trophic function in early fetal life. The condition has an autosomal recessive pattern of inheritance, with morphologic expressivity in hetero-zygotic carriers. The possible relationship of these cases to others with similar morphological patterns is discussed.

While some reports deal with the passage of exogenous insulin in the cerebrospinal fluid (CSF), similar data about endogenously secreted insulin are limited and concern to the best of our knowledge only instantaneous observations on insulin concentrations in the serum and CSF. In the present study, serial changes in CSF levels of insulin after an oral glucose load in 3 patients are reported; some additional data provided from a further 5 patients are also included.
