Dehiscence-type bony defects may occur after implant application because of microbial action as well as of biomechanical and occlusal overload. The aim of the treatment of a periimplant defect is to arrest the progression of the bone loss and to achieve a maintainable site for the implant. In these situations, barrier membranes and bone graft materials can be used to achieve complete bone healing around dental implants. Bone regeneration is possible in a periimplant bony defect of a functioning implant if the proper surgical technique is utilized and the etiologic cause is eradicated. This study presents the surgical coverage of a periimplant bony defect around an implant that was inserted 7 years ago. The surgical correction was made using a barrier membrane in conjunction with bone graft materials. A follow-up of 6 months seemed to reveal radiographic bone regeneration.

Access to accurate clinical data on the frequency of prosthodontic complications encountered in dental implant restorations can provide dentists with important information for evaluating dental implant systems. The complication of prosthetic screw loosening, for example, has often been ignored or under-reported. Only limited published clinical data exists on the use of the Spline connections for dental implant restorations. This paper reports on anin-vitroinvestigation of prosthodontic complications encountered with the Spline dental implant system restored by students and residents at The Ohio State University’s School of Dentistry. From a comprehensive list of implant patients generated from the University’s computer records, an extensive chart review was conducted, and all patients who were treated with Spline dental implants were entered into the study. During a second chart review, data on all prosthodontic complications with Spline implant restorations were gathered on reporting forms then entered into a computer database and subjected to statistical analysis. The results showed that 95.75% of the prostheses were free of any complications. Iatrogenic factors that resulted in screw loosening were identified. Based on the findings of this relatively short-term study ranging from 29 to 59 months of post-restoration clinical follow-up, Spline dental implants provided a stable implant prosthetic connection.

Restoration of single-stage implants with solid abutments traditionally requires the use of an impression index cap, an analog and a lab fee commensurate with a more complex restoration. A technique is discussed that utilizes standard crown and bridge techniques that simplifies the lab procedure and reduces the prosthetic costs involved with fabrication of implant borne crowns.

摘要:
A biologically derived, parabolic implant design that has an occlusal platform that mimics the cementoenamel junction of natural dentition is proposed and described. The interproximal areas of the platform are more coronal than the facial and lingual areas of the platform, and follow the anatomical contours of the alveolar bone. The principle behind this parabolic margin, as opposed to a flat and mono-plane margin, is that any bone re-modeling that follows would replicate a parabolic form. This design helps to maintain bone more coronal on the proximal surfaces than on the facial and lingual surfaces, hence preserving the bone support of the gingival papillae.

PurposeAlthough the efficacy of PepGen P-15 particulate has been conclusively demonstrated in two multicenter clinical studies, the effect of a new hydrogel formulation containing PepGen P-15 on bone repair has not been previously demonstrated.Materials and MethodsPepGen P-15 Flow contains PepGen P-15 particulate in a biocompatible hydrogel, which provides spacing between the particles. In this case study, bone repair of both PepGen P-15 particles and PepGen P-15 Flow was compared to natural bone in same-mouth extraction sockets.ResultsThe radiographic, histologic, and histomorphometric evaluations showed enhanced bone formation and faster particle resorption with PepGen P-15 Flow compared to the PepGen P-15 particulate.ConclusionThe accelerated bone formation by PepGen P-15 Flow may provide faster implant placement.

PurposeThe overall aim of this project was to study osteoblast cell attachment on titanium surfaces with varying surface roughness.Materials and MethodsCommercially pure titanium surfaces were prepared by polishing through 600-grit sandpaper, sandblasting, or sandblasting followed by acid etching to produce surfaces of varying roughness, as determined by scanning electron microscopy and atomic force microscopy.In vitrocell attachment of MC3T3-E1 osteoblasts was performed on the prepared surfaces in both serum-containing and serum-free media conditions.ResultsCell attachment was directly related to the average surface roughness, with the highest levels of cell attachment observed on sandblasted and sandblasted-acid-etched surfaces. Similar patterns of cell attachment were observed when serum-free conditions were employed.ConclusionsCombined surface analytical and cell/molecular biological techniques are powerful tools to broaden our understanding of biological events occurring at the implant-tissue interface. Data acquired from thesein vitrotechniques provide a translational application toin vivoclinical models leading to the next generation of dental implants.

PurposeThe purpose of this pilot study was to evaluate the histologic and mechanical healing process in dental implants under the action of pulsed electromagnetic field (PEMF).Materials and MethodsForty-eight commercially pure implant fixtures were implanted in tibiae metaphysis of 12 New Zealand white rabbits divided into experimental (PEMF) and control groups. A PEMF with pulse width of 85 &mgr;s and a pulse frequency of 20 Mc was applied for 30 minutes per day. The animals were killed 21 and 42 days after implantation. The mechanical tests were performed in all animals and bone biopsies were prepared for decalcified sections analysis.ResultsMechanical tests did not show significant differences between the groups (P> 0.05); however, statistically significant differences were observed over time (P< 0.0001). Similar histologic features were achieved for both groups.ConclusionsThese results suggest that PEMF stimulation does not improve the bone-healing process around commercially pure dental implants.
