Various treatment options have been advocated to restore short span edentulous spaces. For a single edentulous space, some clinicians have advocated the use of a unilateral removable partial denture because this design has certain advantages for the patient, and it does not require the preparation of adjacent teeth for the fabrication of a fixed partial denture. However, potentially serious consequences associated with this design type have been reported. The unilateral partial denture can become dislodged, swallowed, or aspirated, which can result in hospitalization, perforation of the gastrointestinal tract, possibly followed by surgical intervention to retrieve the prosthesis. Although an implant-supported single tooth replacement has been widely accepted and documented as a predictable treatment alternative, few clinical cases present have been presented in which potential severe iatrogenic damage can be so easily avoided. A treatment alternative that avoids potential hazards inherent in the design of a unilateral partial denture has been presented in this article.

摘要:
Patients can present with a variety of conditions that limit, or prevent, the ideal placement of endosteal implants. One of these situations is depicted by an insufficient amount of bone for implant placement in the maxillary anterior alveolar ridge. Autogenous bone grafting can be used to augment the ridge to provide a sufficient amount of bone for implant placement. Using the mandibular symphysis as a donor site offers ease of access, good bone quality for localized repair, a corticocancellous block graft morphology, low morbidity, and minimal graft resorption. A procedure for harvesting and placing an autogenous mandibular bone graft is presented; as are the advantages of using this location as a donor site compared with other locations.

Patients who are to receive implant therapy and are about to undergo radiation treatment require a means to eliminate, or reduce, the amount of radiation received by the implants and their osseous housing. This requirement is even more important if the implants were recently inserted and osseointegration (Sigma) has not yet been achieved1. Lead shielding of the implant sites protects the fixtures and the surrounding bone so that osseointegration can be achieved and maintained.

Very little has been presented in the scientific literature regarding the ability of practitioners to salvage the ailing dental implant once bone loss has occurred. The purpose of this study was to follow patients who had bone loss and increased probing depths associated with their dental implants after initial integration and restoration. All patients either were referred for the treatment of these implant problems or were patients on supportive periodontal therapy in this periodontal practice. Forty two ailing/failing implants in 21 consecutive patients (ages 22–87) all having signs of excessive bone loss and bleeding on probing (most having suppuration) were treated either resectively or via hard or soft tissue augmentation and were followed for 42 to 84 months (average 54 months) in this retrospective study.

This case report describes the use of a single implant as an abutment for a removable cast partial denture. The patient had only anterior teeth remaining in the mandible. One single-stage implant was placed to be used as an abutment for a cast removable partial denture. The periapical X-ray taken 14 months after initial loading showed no bone loss. Clinically, there was no crown loosening, no pocket depth, nor any implant loosening at 14 months. A long-term multicenter study is advocated to evaluate a greater number of patients over a 3− to 5-year time period to evaluate the success of implant-supported crowns when they are used as partial denture abutments.

This article introduces the use of a new bone graft material. The material is a human collagen allograft resorbable matrix impregnated with demineralized freeze-dried bone granules. This material is harvested from the same donor. A series of three cases of extractions with immediate implant placement accompanied by osseous defects were used to demonstrate the use of this material. Bone cores were harvested at Stage II uncovering of the implants. The histologic and clinical results are reviewed.

Most endosseous implant systems are placed using a two-stage surgical procedure. The surgical stages are separated by a period of 4 to 6 months to allow osseointegration. At Stage-I surgery, the implant is fixed in bone and submerged under mucosa, while at Stage-II surgery, the implant is exposed and the transmucosal abutment is connected to the implant. The one-stage nonsubmerged surgical procedure with immediate transmucosal extension using traditional two-stage implant systems is being used to treat partial and complete edentulism. This is a case report of 10 patients treated with endosseous implants in the maxillary anterior region with immediate provisionalization using a one-stage surgical procedure. The investigators found that all patients experienced minimal discomfort from complications after the one-stage surgery and immediate placement of the provisional restoration. No signs of inflammation or mobility were observed during the first 6 months. The partially edentulous anterior maxilla can be restored to an esthetic form in a single clinic visit.

Computerized tomography (CT) scanning, now widely used for the diagnosis and treatment planning of dental implant sites, can be more specifically useful with integrated computer software. In this instance, the potentially critical incisive foramen area at the premaxilla is analyzed to better under stand its configuration and volume relative to the placement of root-form implants.
