An electromechanical tremor scoring device (TSD) that provides an immediate score of tremor severity was compared with clinical assessment in 18 multiple sclerosis (MS) patients with tremor, and 8 normals. The TSD consists of a hinged brace that allows only wrist motion, a potentiometer that serves as a wrist angle transducer, and a light-emitting diode display. On the display appear two linear traces: a pseudorandomly moving target and a wrist transducer signal. The subject attempts to align the wrist with the target signal for 100 seconds. The device determines the absolute voltage difference between the signals, producing an error score. The device is compact and portable. Clinical assessments were performed independently by two neurologists; clinical characteristics of tremor severity were rated and a composite score was obtained. TSD scores and clinical ratings were compared. TSD scores differentiated MS patients (mean, 414.1) from normals (mean, 143.3). The test-retest reliability of the TSD was 0.92. The inter-rater reliability of the clinical composite was 0.74. The correlation between the TSD and the clinical composite was 0.72. Results suggest that the TSD may provide a quick, valid, and reliable technique for tremor measurement.

Pelvic-positioning straps are usually used to maintain the client/seating system interface. This article discusses the proper use of these straps. Proper selection and use of a pelvic-positioning strap will affect the success of the entire seating system, so careful attention must be paid to the size of the strap(s) and components and the placement of the strap as it crosses over the client's thigh/lower pelvic area. The direction of pull as the fastening system is closed and tightened may affect pelvic position as well. The author suggests the use of belts mounted to cross the upper thigh at a 90° angle to the seating surface. Placing the pelvic positioning strap at this angle leaves the pelvis free for active anterior movement during weight shift and functional reaching. Adjustable mounting hardware is recommended as a method of providing a pelvic-positioning system that is flexible enough to achieve proper alignment.

As a major payment source for rehabilitation equipment, Medicaid requires that equipment users, family members, providers, and others who deal with or work within the Medicaid system understand how the system works and the types of equipment for which it might reimburse. This paper analyzes some of the general policies and practices of the Medicaid system, reporting significant findings of a survey conducted of agency officials across the 50 states and the District of Columbia.
