This review addresses some of the challenges confronting the modern nutrition support clinician in developing protocols for nutrition assessment. While it is generally agreed upon that patients who are malnourished are at greater risk for development of complications during hospitalization, there is no consensus on the best method for assessment of nutritional status. Assessment parameters currently available include clinical, biochemical, anthropometric, and functional tests designed to evaluate nutrition status as well as estimate body composition. As some of these parameters are expensive or not practical for routine clinical use, they should be evaluated carefully when a nutrition assessment protocol is designed.

Bone marrow transplantation (BMT) has been associated with numerous negative side effects, which include prolonged anorexia. Historically, parenteral nutrition has been the method of choice for nutrition support. The possibility that enteral feedings may be a practical form of nutrition therapy for those BMT patients who have prolonged anorexia and who have adequate gastrointestinal function was investigated. RR, a 10‐year‐old male, was diagnosed with acute lymphocytic leukemia in March 1989. The patient received a bone marrow transplant in July 1992 from a human leukocyte antigen genotype‐matched sibling. Because of the patient's prolonged anorexia, he was started on enteral feedings via a percutaneous endoscopic gastrostomy feeding tube on day +63. Over the next 300 days of enteral nutrition support the patient's nutritional status improved as demonstrated by an increased weight and height and the maintenance of normal visceral protein stores. This suggests that enteral feedings may be beneficial for those BMT patients who have prolonged anorexia and have adequate gastrointestinal function.

Much controversy exists regarding the nutritional care of obese, hospitalized patients. There is no real agreement among health care professionals in the nutritional support of obese patients. A survey was designed to determine the methods used to provide for the nutritional needs of obese, hospitalized patients. The survey included questions on definition of obesity, equations used to calculate energy and protein needs, and body weight used for calculations. The information received from this survey describes the variability of nutritional care for obese, hospitalized patients. In addition, a review of the current literature is provided regarding the metabolic response of the obese.
