摘要:
This review considers the development of structured triglycerides as an IV nutrient source. The replacement of a portion of long‐chain fatty acids in the conventional fat emulsion triglycerides confers favorable qualities on these lipids and renders them attractive as an energy source, particularly in critically ill patients. This report considers the features of structured triglycerides that may make them competitive or even more efficient as an energy source compared with conventional fat emulsions.

摘要:
The specialty of nutrition relies heavily on trained practitioners to assess patient nutritional status, define nutritional goals, and recommend therapeutic strategies to achieve those goals. To facilitate the timely delivery of nutrition care, the dietitians at the University of Massachusetts Medical Center expanded clinical privileges. With the support of the Nutrition Department, the multidisciplinary Nutrition Support Service, and the Nutrition Advisory Committee, dietitians write orders for diets, oral and parenteral nutrient supplementation; and custom mix parenteral nutrition solutions, tube feedings, transitional feedings, balance studies, blood glucose monitoring, and home care orders (parenteral and enteral). Dietitians also order laboratory tests such as chemistries, indirect calorimetry, and bone densitometry. Clinical privileges are a permanent part of each dietitian's appointment to the Health Professional Staff and are granted by the Chancellor/Dean of the Medical University.

摘要:
Enteral nutrition has become the preferred route of nutrient administration. Because of vigorous attempts to deliver nutrient enterally in expanded patient groups, many different locations for enteral access have been advocated along with a variety of methods related to rate and pattern of delivery. Because all modes of delivery are not compatible with all sites of access and both need to be tailored to specific subsets of patients, confusion can develop regarding where and how enteral nutrients are best delivered and why. In an era when such a high priority has been placed on feeding through the enteral route, a review of the methods and rationale behind the ever‐expanding choices of enteral access is timely.

The use of total nutrient admixtures (TNA) has increased dramatically in both the clinical and home care setting since 1983. The popularity of these dosage forms rests in supplying all the nutrients in one container that meets the nutritional goals of the patient. In addition, TNA formulations obviate the need for either separate peripheral venous catheters or co‐infusions into the primary venous line, thus simplifying this complex therapy. Moreover, the clinical benefits derived from this practice are related to a reduction in glucose‐associated complications that assume particular importance in the acutely ill patient. Although much experience has been gained over the last 12 years, practice standards vary with the institution. Despite this, TNA therapy has been successful in the U.S. This paper attempts to delineate the important physicochemical variables that influence emulsion stability as they relate to clinical practice.

摘要:
The Nutritional Support Service (NSS) at William Beaumont Hospital has successfully inserted over 2000 peripherally inserted central catheters (PICCs) since the inception of its PICC program in 1990. The following is a case study of a 43‐year‐old female with diabetes mellitus and multiple drug allergies who presented to the NSS after Wound Clinic personnel were unable to pull a 43 cm PICC more than 38 cm out of her left arm. Radiographic studies showed the remaining 5 cm of the catheter to be untangled, following the course of the cephalic vein. A surgical cutdown was performed to dilate the cephalic vein and remove the device. The PICC tip was encapsulated in an organic material which was confirmed by pathology to be a thrombus with endothelial lining.
