摘要:
AbstractA survey of 376 women who met the DSM III criteria for bulimia is presented. The purpose of the survey is to provide information regarding the demographics of this population, the total extent of food‐related behavior and basic indices of psychiatric symptoms. The results indicate that the typical bulimic surveyed was a single,Caucasian woman in her early 20's who was well educated and of average weight for her height. Most of the women were binge eating and using evacuation techniques such as self‐induced vomiting on a daily to more than daily basis and had engaged in this behavior for 51/2 years. AbstractThis paper proposes that anorexia nervosa patients are a psychodynamically heterogeneous group and can be usefully divided into three subtypes — the borderline; the empty, understructured; and the emotionally‐conflicted, identity‐confused. Clinical experience and the long‐term outcome literature challenge any uniformity assumptions about anorexia nervosa. AbstractResearch bearing on the psychoneurotic and psychosomatic theories of obesity is reviewed. The extent to which obese persons have been found to be suffering from a mental disorder or to evidence either a distinctive pattern of personality features or a characteristic emotionality is examined. The published reports on the mental status of obese patients in mental health settings show that many of these persons suffer from a neurotic or characterological disturbance involving issues of nurturance, growth and dependence. Obese general medical and nutrition clinic patients have been found to be psychologically disturbed in some cases and free of disturbance in other cases. Controlled research shows that a large proportion of these patients are indistinguishable from normal, nonobese persons on a variety of psychological measures. Another large group of obese medical patients have been found to suffer from various mild mental disturbances. Research comparing the mental status of obese and nonobese persons not under any form of medical care (e.g., government and factory workers) provides compelling evidence that obese persons who are unselected with respect to physical health and psychiatric status function as well or better than comparable nonobese persons. Although the research does not support a psychology of obesity based on psychoneurotic or psychosomatic process, the role of other behavioral and social influences in energy consumption and weight regulation merits careful investigation.

AbstractBased on a clinical study of 80 patients, the aetiology, pathogenesis and clinical features of the Bulimic Syndrome is presented. The use of the term in patients at normal body weight who have had previous Anorexia Nervosa is questioned, not least as the pathonomonic psychological features of Anorexia Nervosa remain. Rather, the view is advanced, with supporting social evidence, that the majority of bulimic patients at normal body weight are a variant of massive, but thwarted obesity. The condition is divided clinically into three groups—a “neurotic” group, a “personality disordered” group and an “epileptiform” group: clinical divisions which have important implications for treatment. Data is presented showing that there is a tendency to move Social Class by education and employment prior to illness and of disturbed menstruation. This latter feature seems to be the result of a fluctuating carbohydrate intake. It is suggested that there is a sensitive switch, possibly hormonal, linking carbohydrate ingestion

AbstractThe study described is an exploratory investigation of personality disorders and level of personality disorganization of a small sample of patients with a diagnosis of anorexia nervosa based on DSM‐III criteria. The focus is on the question of homogeneity or heterogeneity of personality among the subjects. The subjects were six female inpatients aged 13‐19 in a hospital eating disorders program, chosen sequentially from admissions during a five month period. Measures used were: 1) Psychiatric and Social History forms, 2) Diagnostic Interview for Borderlines, 3) WAIS or WISC‐R and 4) Rorschach. Each patient was also given an independent personality diagnosis. All measures were administered when patients were at “target” weight. Psychological test data were scored by two “blind” judges utilizing Johnston and Holzman's Thought Disorder Index. Results indicate that the patients manifest a variety of personality disorders and levels of personality organization. There are some indications that patients with a history of bingeing and vomiting are more likely than the others to meet criteria for borderline personality disorder and borderline level of personality

AbstractTwelve adolescents hospitalized with a diagnosis of anorexia nervosa had their diet and nutritional history evaluated 5 years later. Detailed dietary history was obtained for the previous one year using a standard dietary questionnaire and compared to a matching control group. At follow‐up the average age of anorexics was 19.7 years and controls 20.8 years. Average weight was the same in anorexics and controls. There were no statistically significant differences in intake of key nutrients, percent of the minimum daily recommended allowance of each nutrient ingested, and food source of calories between anorexics and controls. Both groups exceeded recommended allowances for all nutrients except iron. High levels of vitamin A and ascorbic acid in the diet indicated frequent use of fruits and vegetables by subjects and controls. Unlike previous studies results did not indicate carbohydrate avoidance or abnormal eating behavior. Continued inadequate intake was not typical of these young women with a past history of anorexia nervos
