AbstractA 20‐item measure of the cognitive correlates of bulimia, the Bulimic Thoughts Questionnaire (BTQ), is developed. Confirmatory factor analysis supports a threefactor structure for this measure: Self‐Schema, Self‐Efficacy, and Salient Beliefs. Study 1 compares bulimic, obese, and normal women on these factors. Bulimics demonstrate larger body image (Self‐Schema), lower expectations regarding ability to maintain desirable weight (Self‐Efficacy), and greater frequency of thoughts relevant to bulimia (Salient Beliefs) than do normals. Bulimic and obese women do not differ on Self‐Schema and Self‐Efficacy measures, even though objectively the obese group weighed, on average, 40 Ib more. AbstractThe purpose of this single case study of a bulimic patient was to replicate and incorporate certain elements of bulimic behavior, namely fasting, carbohydrate loading, vomiting, and binge eating, into the glucose tolerance test in order to observe the effects of these modifications on plasma insulin, noradrenalin, and cortisol. The most striking finding was that of potentiation of insulin secretion in response to a glucose load most marked after the subject had engaged in binging and vomiting. The results support the hypothesis that engagement in bulimic behaviors induces higher plasma insulin levels and that this precipitates a behavioral vicious cycle with associated mood changes. Other applications of this experimental model are discussed.

AbstractThis study examined the personality organization and functioning of 57 normal weight bulimic women as assessed by the Rorschach and compared their performance to that of comparable groups of nonpatient and outpatient depressed women. In contrast to nonpatients, the bulimic and depressed groups were similar in their overall level of dysphoric affect, emotional lability and impulsivity, avoidance of affective stimulation, state of emotional overload, relatively poor perceptual accuracy, lower interest in other people, and lack of set coping style. The bulimic group, however, displayed greater egocentricity, narcissism, anger, and negativity and was more underincorporative than the depressed, who were more introspective but also more arbitrary in their perceptions than the bulimics.

AbstractDifferences in the variability of the judgements of body shape were studied in anorexic, bulimic, obese, and control subjects. The judgements of control subjects were found to be more accurate and less variable than those of the clinical groups, and the distribution of their scores was leptokurtic and almost symmetrical compared with the clinical groups' distributions of scores which were platykurtic and positively skewed. More clinical subjects than controls both underestimated or overestimated their body sizes by 15% or more. Significantly more variability was found among the scores of bulimics than anorexics and among the scores of the obese than the anorexics, but not between the obese and the bulimics, who showed the greatest variability of all. The results are discussed in terms of their consistency with other research findings and their significance for treatment.

AbstractThe notion of using weight tables to categorize patients with eating disorders is at best simplistic and at worst inappropriate. Available tables suffer from serious conceptual, sampling, and methodological deficiencies and are especially unsuited for use with young, white middle‐class females: the population at particular risk for eating disorders. AbstractThe secretion pattern of luteinizing hormone (LH) and follicle‐stimulating hormone (FSH) was studied in 15 normal‐weight patients with bulimia over a 12‐hour period during the early follicular phase of the menstrual cycle. AbstractPregnancies of two women with a history of anorexia nervosa are described. Anxieties and fears of obesity were greatest during the first trimester and the second postpartum month. Disturbed eating behaviors gradually diminished during pregnancy but were augmented approximately 6 weeks after delivery. One woman received regular emotional support and dietary counseling from obstetrical staff during her pregnancy. Neither woman received counseling during the postpartum crisis. Supplemental antenatal and postnatal counseling for women with eating disorders is recommended.

AbstractThis paper examines the role of childhood sexual abuse antecedent to anorexia nervosa, bulimia, and compulsive overeating. Three case reports are used to illustrate the development of eating disorders in the context of sexual abuse. Implications are discussed in terms of the need for professionals to develop sensitivity to this potential antecedent in order to apply appropriate intervention strategies with sexual abuse victims seeking treatment for an eating disorder.
